Özet
Maternal ölüm sonrası doğurtulan fetusların sonuçlarını araştırmak amacıyla hastanemizde görülen iki olgunun dosyaları incelendi. Birinci olgu 37 yaşındaydı ve eklampsi krizi sonrası kardiak arrest gelişti. Annenin ölümünden sonraki 15. dakikada 2900g ağırlığındaki fetus apgar skoru 1 olarak doğurtuldu. Hemen resüsite edilen yenidoğan, postpartum beşinci günde sorunsuz olarak tabucu edildi. İkinci olgu 44 yaşındaydı ve altta yatan kalp hastalığına sekonder kardiak arrest gelişti. Anne ölümünü takiben 25. dakikada vakum ile 4000g fetus apgar skoru sıfır olarak doğurtuldu. Resüsite edilen fetusda herhangi bir konvülziyon görülmedi ve postpartum yedinci günde taburcu edildi. Kardiak arrest sonrası hemen kardiopulmoner resüsitasyon başlanan annelerde cevap alınamadığı düşünüldüğünde postmortem doğum 25. dakikaya kadar planlanabilir.
Abstract
To present and discuss the outcomes of postmortem delivery in two cases in view of the literature. Two cases who were delivered after maternal death are reviewed and perinatal outcomes are discussed. First case was 37 years old and had cardiac arrest after and eclampsi crisis. A 2900g fetus was delivered fifteen minutes after maternal death with an apgar score of one. Immediate resusciation succeeded and the fetus was discharged on the fifth postpartum day with full recovery. Second case was 44 years old and had cardiac arrest due to an underlying cardiac disease. A 4000g fetus was delivered via vacuum extraction twenty-five minutes after maternal death.The Apgar score was zero. The fetus recovered after resusciation and discharged on the seventh postpartum day uneventfully. Postmortem delivery should be considered in patients who had a cardiac arrest within 25 minutes after the arrest as favorable infant survival can be obtained if immeadiate cardiopulmonary resusciation is performed.
