Akreos MI-60 göz içi lenslerinin biaksiyel küçük kesili katarakt cerrahisi ile yerleştirilmesinin değerlendirilmesi

Özet
Amaç: Biaksiyel-küçük kesili katarakt cerrahisi ile yerleştirilebilen Akreos MI-60 göz içi lenslerinin etkinlik ve güvenilirliğinin ortaya konması amaçlandı Gereç ve Yöntem: On-sekiz hastanın 21 gözünde Biaksiyel küçük kesili katarakt cerrahisi (B-KKKC) yöntemi ile Akreos MI-60 (Bausch & Lomb) göz içi lensleri (GİL) yerleştirildi. Olguların yaş ortalaması 55.4±14.1, ortalama takip süresi 9.61±1.46 aydı. Hastaların ameliyatlar sonrası yapılan değerlendirilmelerinde, ön segment cevabı, elde edilen görme keskinlikleri, kontrast duyarlılık ölçümleri ve cerrahi nedenli astigmatizma hesaplamaları yapıldı ve değerlendirildi. Bulgular: Ameliyatlar sırasında bir olgu dışında komplikasyon gelişmedi, bu olguda arka kapsül ruptürüne rağmen kapsül içi GİL’i yerleştirildi. Hastaların ameliyatlar sonrası ortalama düzeltilmiş görme keskinliği ondalık: 0.94±0.11, log-MAR: 0.029±0.059 olarak saptandı, Ortalama kontrast duyarlılık değerleri ameliyatlar sonrası 90. günde 3, 6, 12, 18 cpd’ler için sırasıyla glaresiz; 1.62±0.18, 1.94±0.22, 1.61±0.24, 1.16±0.28 ve glareli; 1.59±0.18, 1.84±0.19, 1.54±0.19, 1.16±0.26, olarak bulundu. Ameliyatlar sonrası ortalama kesi genişliği 1.8±0.09 mm (1.7-2.0 mm), cerrahi uyarılmış astigmatizma ise vektör analiz yöntemi ile 0.14 @ 68º olarak ölçüldü. Biri şiddetli olmak üzere 2 olguda tedavi ile tamamen düzelen ön kamara membranı izlendi ve tüm olgularda GİL’lerinin mükemmelen santralize olduğu gözlendi. Sonuçlar: B-KKKC tekniğinin bize sağladığı avantajları kullanmamıza olanak verecek nitelikteki, dört haptikli hidrofilik akrilat yapılı Akreos MI-60 GİL’i, 1.8 mm ve altı küçük kesiden yapılabilen ameliyatlarda, verdiği olumlu sonuçlarla, bu alanda yeni bir seçenek olarak ortaya çıkmıştır.
Abstract
Purpose: To evaluate the efficacy and reliability of Akreos MI- 60 intra ocular lenses (IOL) which may be implanted with biaxial microincisional cataract surgery Materials and Methods: Akreos MI-60 (Bausch & Lomb) IOLs were implanted in 21 eyes of 18 patients using the biaxial microincisional cataract surgery (B-MICS) technique. Mean age of the patients was 55.4±14.1 years and mean follow - up time was 9.61±1.46 months. For the postopertative assessment of the patients; anterior segment responses, postoperative visual acuities, contrast sensitivities and surgically induced astigmatism (SIA) measurements were accomplished and evaluated. Results: There was no intraoperative complication, except one case in which the IOL could be implanted in the bag, in spite of posterior capsular rupture. Mean best corrected visual acuity was established as 0.94±0.11 (decimal), 0.029±0.059 (log-MAR). Mean contrast sensitivity values were 1.62±0.18, 1.94±0.22, 1.61±0.24 and 1.16±0.28 without glare and 1.59±0.18, 1.84±0.19, 1.54±0.19 and 1.16±0.26 with glare for 3, 6, 12 and 18 cpd respectively at the 90th postoperative day. The post operatively measured average incision width was 1,8±0,09 mm (1.7- 2.0 mm), and SIA was calculated as 0.14 @ 68º with the vectorial analysis method. In two cases, anterior chamber membranes were defined, one of them was severe, but both could be cured completely and the IOLs were perfectly centralized in all cases. Conclusion: With the hydrophilic acrylate material and four haptics design Acreos MI-60 IOL enables us to take advantage of B-MICS technique implanting through 1.8 mm and smaller incision widths. We deduced with these affirmative results that this lens would be a new promising option in the small incision surgery area.
