Özet

Amaç: Nd:YAG lazer arka kapsülotomi sonrası erken dönem göz içi basınç (GİB) artışını önlemede %0.2’lik brimonidinin etkinliğini incelemek. Gereç ve Yöntem: FAKO cerrahisi ve arka kamara göz içi lens implantasyonu sonrası arka kapsül kesafeti gelişen 80 hastanın 88 gözü çalışmaya alındı.Kapsülotomi öncesi hastaların görme ve göz içi basınç ölçümleri, biyomikroskopik muayeneleri yapıldı. Tüm olgulara arka kapsülotomi öncesi GİB kontrolü için %0.5’lik apraclonidin HCl 1 kez damlatıldı. Kapsülotomi sonrası bir gruba (42 göz) ilave olarak %0.2’lik brimonidin, 2x1 başlandı ve 1 hafta devam edildi. Diğer grup (46 göz) kontrol grubu olarak kabul edildi ve herhangi bir antiglokomatöz tedavi uygulanmadı. Hastaların GİB değerleri 1. saat, 1. gün, 1. hafta ve 1. ay ölçüldü. Bulgular: Yaş ortalaması 63.1±23.12 idi. Cerrahi sonrası arka kapsülotomiye kadar geçen süre 2 ile 48 ay (ortalama 22±9.3 ay) idi. Takiplerde brimonidin grubunda işlem öncesi ortalama GİB 15.2±3.7 mmHg iken işlem sonrası 1. saat 16.1±2.2, 1. gün 16.1±2.4, 1. hafta 15.7±3.2, 1. ay 14.1±2.2 mmHg olarak ölçüldü. Kontrol grubunda ise işlem öncesi 14.7±3.2 mmHg iken işlem sonrası 1. saatte 17.7±3.4, 1. gün 18.2±2.7, 1. hafta 16.2±3.4, 1. ay 15.2±2.8 mmHg ölçüldü. Brimonidin grubunda kontrol grubuna göre 1. saat ve 1. gündeki ortalama GİB değerleri arasında istatistiksel anlamlı fark saptandı(p 0.05), kontrol grubunda 1.saat ve 1.gündeki GİB artışı istatistiksel anlamlı idi (p<0.05). Sonuç: Nd:YAG lazer arka kapsülotomi sonrası bir haftalık %0.2 brimonidin uygulaması ile erken dönemde etkili GİB kontrolü sağlanabilmektedir.

Abstract

Purpose: To evaluate the efficiency of brimonidin 0.2%on an intraocular pressure (IOP) increase after Nd:YAG laser posterior capsulotomy. Materials and Methods: Eighty patients (88 eyes) who had posterior capsule opacification after phacoemulsification and posterior chamber IOL implantation surgery were included. Patients&#8217; visual acuity and IOP were measured, and a biomicroscopic examination was performed before capsulotomy. Before the laser surgery apraclonidine 0.5%was applied in all patients. Brimonidin 0.2%was applied twice daily in the first group (42 eyes) after capsulotomy. The other group was the control group (46 eyes); it received no antiglaucomatous treatment. IOP values were measured after 1 hour, 1 day, 1 week, and 1 month. Results: Mean age was 63.1±23.12. The time from cataract surgery to posterior capsulotomy was a mean of 22±9.3 (2-48) months. Before capsulotomy, while the mean IOP was 15.2±3.7 mmHg in the first group, it was 16.1±2.2 after 1 hour, 16.1±2.4 after 1 day, 15.7±3.2 after 1 week, and 14.1±2.2 after 1 month. In the control group it was 14.7±3.2 mmHg before the procedure, while it was 17.7±3.4 after 1 hour, 18.2±2.7 after 1 day, 16.2±3.4 after 1 week, and 15.2±2.8 after 1 month. Statistical significance was determined between mean IOP measurements in the two groups (p<0.05). Moreover, no significant IOP increase was seen in the brimonidine group compared to before surgery (p>0.05). In the control group the IOP increase was statistically significant after 1 hour and 1 day (p<0.05). Conclusion: Topical use of brimonidine 0.2%for 1 week after Nd:YAG laser posterior capsulotomy is effective in preventing IOP elevation in the early period.
