Tedavi ile kontrol altında olan sistemik hipertansiyonun göz içi basıncı ve oküler puls amplitüde etkisi

Özet
Amaç: Sistemik hipertansiyonu olan hastaların göz içi basınçlarını (GİB) ve oküler puls amplitüdünü (OPA) normal bireylerle karşılaştırmak. Gereç ve Yöntem: Tedavi ile kontrol altında olan sistemik hipertansiyonlu 33 hasta ile kontrol grubu olarak 24 sağlıklı birey çalışmaya dâhil edildi. Tüm bireylere 24 saatlik ambulatuar kan basıncı ölçümü, dinamik kontur tonometre ile göz içi basıncı ve OPA ölçümü yapıldı. Gruplar; GİB, OPA, ortalama sistolik kan basıncı (SKB) ve ortalama diyastolik kan basıncı (DKB) açısından istatistiksel olarak değerlendirildi. Bulgular: Sistemik hipertansiyona sahip 33 hastanın (23 kadın/ 10 erkek) yaş ortalaması 53.0±11.7 yıldı. Bu grupta ortalama GİB; 16.8±2.1 mmHg, OPA; 2.9±1.0 mmHg, SKB; 125.9±10.8 mmHg ve DKB; 81.3±9.9 mmHg olarak saptandı. Kontrol grubunda 24 bireyin (14 kadın/10 erkek) yaş ortalaması 49.9±10.3 yıldı. Bu grupta ortalama GİB; 16.1±1.9 mmHg, OPA; 2.5±0.7 mmHg, SKB; 122±8.7 mmHg ve DKB; 81.0±7.3 mmHg olarak saptandı. Her iki grupta, OPA haricindeki parametreler açısından istatistiksel olarak anlamlı bir fark yoktu. Sonuç: Hem kontrol altındaki sistemik hipertansiyonu olan hastalar hem de normal bireyler aynı SKB, DKB ve GİB’e sahip olmalarına rağmen OPA değerleri hipertansiyon grubunda daha yüksek olarak ölçülmektedir. Bu durum sistemik hipertansiyonlu hastaların koroidal perfüzyonunun ve oküler kan akımlarının normal bireylerden daha fazla olduğunu göstermektedir.
Abstract
Purpose: To compare the intraocular pressure (IOP) and ocular pulse amplitude (OPA) between the patients with systemic hypertension and normal individuals. Materials and Methods: Thirty-three patients with controlled systemic hypertension with treatment and 24 healthy subjects were included in the study. All participants underwent 24-hour ambulatory blood pressure monitoring, IOP and OPA measurements with dynamic contour tonometry. Statistically analysis of the groups was also performed according to mean IOP, OPA, systolic blood pressure (SBP) and diastolic blood pressure (DBP). Results: The mean age of the 33 patients (23 female/10 male) was 53.0±11.7 years in hypertension group. Mean IOP was 16.8±2.1 mmHg, OPA was 2.9±1.0 mmHg, SBP was 125.9±10.8 mmHg and DBP was 81.3±9.9 mmHg in this group. In control group, the mean age of the 24 subjects (14 female / 10 male) was 49.9±10.3 years. Mean IOP was16.1±1.9 mmHg, OPA was 2.5±0.7 mmHg, SBP was 122±8.7 mmHg and DBP was 81.0±7.3 mmHg in this group. There was no statistically significant difference between the groups in terms of the other parameters except for OPA. Conclusion: However, patients with controlled systemic hypertension had higher the OPA values than normal individuals, both groups had similar the SBP, DBP and IOP values. The elevated OPA values in the patients with systemic hypertension are most likely secondary to increased choroidal perfusion and ocular blood flow.
