Özet

Bu çalışmada görülme sıklığı giderek artan toksik anterior segment sendromunun (TASS) nedenlerini, klinik bulgularını ve önlenmesi için alınması gereken tedbirleri olgularımız rehberliğinde değerlendirmeyi amaçladık. Nisan 2010’da aynı gün elektif şartlarda Fakoemülsifikasyon ve göz içi lens implantasyonu yapılmış ardından TASS gelişmiş 4 olgunun bulguları ve takip sonuçları değerlendirildi. Tüm olgularda ameliyat sonrası ilk gün limbustan limbusa uzanan yoğun kornea ödemi, yaygın Descement’in Membranı kırışıklığı, ışığa yanıt vermeyen middilate, düzensiz pupil mevcuttu. Bir olguda göz içi basıncı (GİB) yükselmişti diğerinde ise takiplerde yükseldi. Endoftalmiyi düşündürecek kapak şişliği, yoğun hiperemi ve kemozis, ağrı, sekresyon hiçbir olgumuzda yoktu. Tüm olgularımızda kornea ödemi kalıcı oldu ve penetran keratoplasti gerekti. Bir olgumuzun GİB’ı ilaç tedavisi ile kontrol altına alınırken diğeri için cerrahi gerekti. TASS ön segment cerrahisi sonrasında ilk 12-48 saat içinde gelişen, endoftalmi benzeri klinik bulgu gösteren, özellikle endotel yıkımına neden olan klinik durumdur. Cerrahide kullanılan her türlü madde TASS’a neden olabilirken en sık neden uygun yapılmayan dezenfeksiyon ve sterilizasyon protokolleridir. Tedavi öncesinde endoftalmi ile ayırıcı tanısı yapılmalıdır. En önemli nokta özellikle göz cerrahi merkezlerin hızla arttığı günümüzde önlenmesi için gereken bütün tedbirleri almaktır.

Abstract

Aim of the study is to evaluate the reasons, clinical findings and precautions that should be taken to prevent toxic anterior segment syndrome (TASS), which has an increasing frequency, under the guidance of our cases. In April 2010, phacoemulsification-intraocular lens implantation was performed on the same day and findings as well as follow up results were evaluated for 4 cases with TASS. All of the subjects had dense corneal edema from limbus to limbus, extensive Descemet’s fold, and mid-dilated, non-reactive pupil, in the first post-operative day. One subject’s intraocular pressure (IOP) was high first post-operative day and another had following days. None of the subjects had eyelid swelling, extensive pain, conjunctival injection and chemosis, mucopurulent exudate which suggested endophthalmitis. All of the subjects had persistent corneal edema and keratoplasty was required. IOP could be controlled in one subject with medication, whereas the other subject needed surgery. TASS is a clinical condition which is developed within 12-48 hours after surgery, has endophthalmitis-like clinical symptoms, and particularly causes endothelial cell damage. Even though any of the device or substances used in the surgery can cause TASS, inappropriate sterilization procedures or maintanance after sterilization are among the most frequently cited reasons for TASS. Endophthalmitis should be considered in the differential diagnosis of TASS before the treatment. The most important point is to take all precautions to prevent TASS, which has a high ocular morbidity especially nowadays, when number of the surgical centres are getting increased.
