Özet

Amaç: Cerrahi öncesi glokomu bulunmayan, psödoeksfolyas- yon sendromu (PES) olan ve olmayan hastalarda, Fakoe- mülsifikasyon (FE) ve göz içi mercek (GİM) uygulanması sonrası erken dönemde göz içi basıncı (GİB) artışının ince- lenmesi ve artan göz içi basıncına 1. günde yapılan müda- halenin etkinliğinin araştırılması amaçlanmıştır . Gereç ve Yöntem: Çalışma Nisan 2004-Mayıs 2004 tarih- leri arasında Ankara Eğitim ve Araştırma Hastanesi Göz Kliniği’nde FE+GİM uygulanan, glokom tanısı olmayan 120 olguyla yapılmış prospektif bir çalışmadır. Olgular PES olan (Grup 1) ve olmayan (Grup 2) şeklinde iki gruba ay- rıldı, cerrahi sonrası 3-7. saatler arasında, 1. günde ve 7. günde muayene edildi; GİB artışı, yara yeri sızıntısı, üveit ve endoftalmiyi içeren komplikasyonlar açısından değerlendi- rildi. GİB artışı olan olgularda uygulanan tedavi yöntemleri ve etkinliği araştırıldı. Bulgular: Tüm olgularda cerrahi sonrası 3-7. saatlerde GİB’ nda artış tespit edildi (p 0.05) Hiç bir olguda üveit, endoftalmi ya da yara yeri sızıntısına rastlanmadı. Sonuç: Tüm olgularda 3-7. saatler arasında GİB’ nda artış tes- pit edilmiştir. Özellikle PES bulunan olgularda FE sonrası ani GİB artışı daha sık görüldüğünden bu olguların erken dönemde GİB artışları açısından sıkı takibinin önemine dikkat çekilmektedir.

Abstract

Purpose: To evaluate the incidence of intraocular pressure (IOP) elevation in the early period after phacoemulsifica- tion (PE) and intraocular lens implantation (IOL) in non- glaucoma patients with and without pseudoexfoliation syndrome (PES), and to determine the effectiveness of dif- ferent types of management. Materials And Methods: In this prospective study, 120 non- glaucoma patients who underwent PE+IOL implantation in Ankara Education and Research Hospital from April 2004 to May 2004 were included. Patients with PES (Group 1) and patients without PES (Group 2) were examined on the day of surgery (3rd-7th hour), on the 1st and 7th postopera- tive days. The main outcome measures were the incidence and management of complications, including IOP elevati- on, wound leaks, uveitis and endophthalmitis. Results: There was a significant elevation in IOP from the ba- seline values, in postoperative 3rd-7th hours, in all patients (p<0.01). This elevation was greater in Group 1 (p<0.05). IOP was more than 28 mmHg in four patients (3.3%) on the 1st postoperative day. On the 7th post-operative day, 50% of these patients had an IOP&#8804;21 mmHg, after topi- cal anti-glaucomatous medication and anterior chamber paracentesis. The elevation in IOP on the 1st day and 7th postoperative day did not differ between Group 1 and 2. None of the patients had uveitis, endophthalmitis or wound leak. Conclusion: All of the patients had a rise in IOP in the 3rd-7th postoperative hours. It is emphasized thatV patients with PES are more prone to early rises in IOP after PE, and they should be carefully monitored after surgery.
