Trabekülektominin kornea endotel hücrelerine etkisi

Özet
Amaç: Trabekülektomi ameliyatının kornea endotel hücreleri üzerine etkisini ve ilişkili faktörleri incelemek. Gereç ve Yöntem: Trabekülektomi geçirmiş 36 hastanın 39 gözü prospektif olarak incelendi. Yaşları, önceden geçir- dikleri göz içi ameliyatları, psödoeksfolyasyon ve diyabe- tes mellitus varlığı, glokom tipi,intraoperatif mitomisin C (MMC) ve ameliyat sonrası 5-Florourasil (5-FU) kullanılıp kullanılmadığı ve ameliyat sonrası komplikasyonlar kayde- dildi. Ameliyat sonrası ve ameliyat sonrası 1. ay, 3. ay ve 6. ayda en iyi düzeltilmiş görme keskinlikleri, göz içi ba- sınçları (GİB), speküler mikroskopla ölçülen endotel hücre büyüklüğü ve mm2’deki endotel hücre sayısı karşılaştırıldı. Bulgular: Ameliyat öncesi ortalama endotel sayısı 2167.2±551.1 iken ameliyat sonrası 1. ayda 2119.5±618, 2; 3. ayda 1908.8±661.8 ve 6. ayda 2163.7±658.4 bu- lunmuştur. Endotel hücre sayısında ve ortalama endotel hücre büyüklüğünde de ameliyat öncesiile ameliyat son- rası takipler arasında istatistiksel anlamlı değişim olmadı (p>0.05). Ameliyat öncesine göre ameliyat sonrası tüm kontrollerde anlamlı GİB düşüşü elde edildi (p<0.05). Ön- ceden göziçi cerrahi geçirmişlerde ve dar açılı glokomlu- larda ameliyat öncesidaha düşük endotel sayısı tespit edil- miştir. İntraop MMC ve ameliyat sonrası 5-FU kullanımının endotele anlamlı etkisi bulunmayıp ameliyat sonrası ön ka- mara sığlaşmasının endotel sayısını azalttığı tespit edildi. Sonuç: Önceden göz içi cerrahi geçirmiş gözlerde, dar açılı glokomlularda ve ameliyat sonrası ön kamara (ÖK) sığ- laşması görülen olgularda trabekülektomi öncesi ve son- rası endotel sayımı yapılarak daha dikkatli takip edilmesini önermekteyiz.
Abstract
Purpose: To evaluate the effect of trabeculectomy on corneal endothelial cells and related factors. Materials and Methods: 39 eyes of 36 patients who had undergone trabeculectomy were evaluated prospectively. Age, intraocular surgery history, pseudoexfoliation, diabe- tes mellitus, glaucoma type, intraoperative mitomycin-C (MMC) and postoperative 5-Flourouracil (5-FU) use and postoperative complications were recorded. Preoperative, postoperative 1st, 3rd and 6th month best corrected visual acuity, intraocular pressure (IOP), endothelial cell area and count per mm2 measured by specular microscope were compared. Results: Endothelial cell count was 2167.2±551.1 preope- ratively, 2119.5±618.2 in the postoperative 1st month, 1908.8±661.8 in the postoperative 3rd month and 2163.7±658.4 in the postoperative 6th month. There were no statistical significant differences between pre and postoperative endothelial cell areas or counts (p>0.05). A statistically significant IOP drop was obtained in all posto- perative controls compared to preoperative IOP (p<0.05). We observed that eyes which had a history of intraocular surgery and narrow angle glaucoma had lower endothelial cell counts preoperatively. We observed that intraoperative MMC and postoperative 5-FU had no effect on endothelial cells but postoperative anterior chamber narrowing decre- ased the endothelial cell count. Conclusion: We suggest that eyes with or intraocular surgery history, narrow angle glaucoma and postoperative anteri- or chamber narrowing should be evaluated more carefully with endothelial cell count before and after trabeculec- tomy.
