Topiramat kullanımına bağlı akut açı kapanması glokomu gelişen pediatrik bir olgunun klinik özellikleri

Özet
Göz hastalıkları polikliniğine bir gün önce başlayan her iki gözdeki görme azlığı ve kızarıklık şikayeti ile başvuran 12 yaşındaki kız çocuğuna ait klinik bulgular değerlendirildi. Baş ağrısı nedeniyle bir hafta önce topiramat tedavisi başlanılan olgunun başvuru esnasındaki tashihli görme keskinlikleri sağda 0.1, solda 0.2 düzeyindeydi. Refraksiyon değerleri sağda -9.00D, solda -8.00 D olan olgunun biomikroskopik ön segment muayenesinde her iki gözde de konjonktival hiperemi ile ön kamarada daralma ve iridokorneal açı kapanması mevcuttu. Göz tansiyonu sağda 28, solda 27 mmHg olan olgunun ultrasonik biomikroskopi (UBM) incelemesinde suprasilyer koroideal efüzyona eşlik eden iris-lens diyaframının öne gelmesi sonucu ön kamarada daralma, iridokorneal açı kapanması, koroid dekolmanı ve silyer cisimde kalınlık artışı mevcuttu. Topiramat tedavisi kesilen olguya topikal antiglokomatöz ilaç, oral karbonik anhidraz inhibitörü ve nonsteroid antienflamatuar ilaç tedavisi başlanıldı. Tedavinin üçüncü gününde tüm göz semptom ve bulgularında düzelme kaydedilen olgunun tedavisi sonlandırıldı.
Abstract
Clinical findings of a 12-year-old girl admitted to our clinic with the complaints of decreased vision and redness in both eyes that had started a day before were evaluated. The patient had been on topiramate treatment during the previous week due to headaches. Corrected visual acuities at the time of admission were 0.1 on the right and 0.2 on the left. Refractive indices were -9.00D on the right and -8.00D on the left. The biomicroscopic examination showed conjunctival hyperemia in both eyes, narrowing of the anterior chamber, and closure of the iridocorneal angle. Intraocular pressure was 28 mmHg on the right and 27 mmHg on the left. In the ultrasonographic biomicroscopy examination (UBM), accompanying supraciliary choroideal effusion there was narrowing of the anterior chamber due to anterior displacement of the iris-lens diaphragm, closure of the iridocorneal angle, choroid detachment, and increased thickness of the ciliary body. The topiramate treatment was stopped. The patient was administered antiglaucomatous drugs, oral carbonic anhydrase inhibitors, and non-steroid anti-inflammatory treatment. On the third day of the treatment, there was improvement in all ocular symptoms and findings, and the treatment was stopped.
