Özet

Amaç: Glokom cerrahisinde, fibroselüler skar formasyonunu önlemek için kullanılan güncel drogların endotel üzerine yan etkileri kullanımlarını sınırlamaktadır. -tokoferolün ise, -doğal bir antioksidan olmanın ötesinde, tenon fibroblast proliferasyonunu ve fibroselüler skar formasyonunu engelleyen, diğer antifibrotiklerin kullandığı yolaklara etkili bir nöromedyatör olduğu bilinmektedir. Bu çalışmada, -tokoferol ve diğer antifibrotiklerin insan endotel hücreleri üzerine in vivo kullanıma uygun dozlardaki apoptotik etkileri farklı kombinasyon ve sürelerde in vitro incelendi. Gereç ve Yöntem: ECV304 İnsan ulimbikal ven endotelyal hücre kültürlerinde antifibrotik ajanların etkileri; 5 dakikalık tek uygulama, ve 10 gün boyunca -tokoferol ilavesiyle kombine olarak incelendi. Antifibrotik ajan olarak paklitaksel, mitomisin, 5-FU ve -tokoferol kullanıldı. DAPI boyama ile floresans mikroskopunda apoptotik indeks tayin edildi. İmmunohistokimyasal yöntem ile hücrelerin kaspaz aktivitelerini incelenerek stoplazmik komponent değerlendirildi ve Reverse Transkriptaz-Polymerase Chain Reaction (PCR) tekniği kullanılarak bcl-2 ve bax gen ekspresyonları araştırıldı. Mitotik indeks tayini yapılarak hücrelerin proliferasyon eğilimleri belirlendi. Bulgular: Kısa ve uzun dönem apoptotik indeks değerleri mitomisin ve özellikle paklitakselde anlamlı yüksek bulundu. Mitomisinin etkisinin uzun dönemde tokoferol ilavesi ile anlamlı derecede arttığı gözlendi. Bcl-2/Bax oranınlarında artış ve kaspaz 3 aktivitesi de sadece mitomisin grubunda görüldü. Uzun dönem örneklerinde ise kaspaz 3 aktivitesi mitomisine ek olarak paklitaksel grubunda da görüldü. Kısa dönem mitotik indeks sonuçlarında -tokoferol, uzun dönemde ise paklitaxel+Tokoferol guruplarında anlamlı artış bulundu. Sonuç: -tokoferol’ün kombine kullanımı antifibrotiklerin endotel hücrelerindeki apoptotik etkilerini azaltmamıştır. Özellikle paklitaksel ve mitomisin ile olan additif etkisi incelenebilir. -tokoferol’ün ve 5-FU’in tek başına ve kombine uzun ve kısa süreli etkileri kontrol grubuna yakın olduğundan, kullanımları endotel toksisitesi açısından daha güvenlidir.

Abstract

Purpose: Side effects current antifibrotics on endothelium, limits their use in glaucoma surgery. The natural antioxidant, - tocopherol is also a neuromediator, and it not only prevents tenon’s fibroblast proliferation and fibrocellular scar formation, but effects similar pathways with those of other antifibrotics as well. In this study, apoptotic effects of -tocopherol and other antifibrotics -in concordant doses with their in vivo uses, on human endothelial cells were examined in vitro in various combinations and durations. Materials and Methods: Effects of antifibrotic agents in ECV304 Human Umblical Vein Endothelial Cell Cultures were evaluated in single 5 minnutes applications and in combination with 10 days of -tocopherol additions. Paclitaxel, mitomycin, 5-FU and -tocopherol were used as antifibrotic agents. Apoptotic indexes were found with DAPI staining in fluorescence microscope. Apoptotic cells were demonstrated immunohistochemically in preperates stained with peroxidase labeled caspase 3 monoclonal antibodies and bcl-2 ve bax gene expressions were evaluated with PCR. Mitotic indexes were determined to evaluate the proliferative trends. Results: Mitomycin and paclitaxel groups demonstrated a significant AI increase, which were augmented by -tocopherol addition. Either bcl-2/bax ratio decrease, or caspase activity were found only in Mitomycin group and further augmentation was achieved with -tocopherol addition. High proliferation was observed in tocopherol and paclitaxel+ tocopherol groups. Conclusion: Combination with -tocopherol did not reduce the apoptotic effects of antifibrotics on human endothelial cells. Single and combined uses of 5-FU and -tocopherol have been found to be safe for their long and short term effects were not statistically different from controls.
