Akut intestinal iskemi tanısında serum D(-) laktat düzeyi ve yüzeyel oksijen basınç ölçümlerinin değerlendirilmesi

Özet
Akut intestinal iskemi ve infarktüsün erken evrede tanısı ve ameliyat esnasında barsak viabilitesinin sınırlarının saptanması cerrahları her zaman zorda bırakmıştır.Bu çalışmada akut intestinal iskemi oluşturulan tavşanlarda K+, SGOT, SGPT, D(-) ve L(-) laktat serum düzeylerinin intestinal iskeminin ön tanısında prognostik değeri ile, standart klinik kriterlerin ve pulse oksimetrenin, barsak viabilitesini belirlemede anlamının olup olmadığı araştırılmıştır. GATA Genel Cerrahi Anabilim Dalında intestinal iskemiye erken evrede noninvaziv tetkiklerle tanı koyabilmek için tavşanlardan intestinal iskemi modelleri oluşturulmuş ve bunlar kontrol gruplarıyla karşılaştırılmıştır. Çalışmada iskemi modelleri oluşturulan deneklerden ve kontrol gruplarından beşinci dakikada, ikinci ve dördüncü saatlerde kan örnekleri alınarak D(-)laktat ve L(-) laktat serum seviyeleri ve dördüncü saat sonunda alınan kan örneklerinden K+, SGOT, SGPT serum seviyelerine bakılmıştır. Deneklerden SMA ligasyonu yapılanlara 4 saat sonra reexplorasyon yapılarak iskemik segmentin viabilite sınırları standart iskemik kriterlere ve pulse oksimetre ölçümlerine göre saptanıp, santimetre olarak rezeke edilen segment kaydedilmiştir. Rezeke edilen bu segmentler histopatolojik olarak değerlendirilmiştir. Elde edilen sonuçlar Mann Whitney U ve Wilcoxon testi ile değerlendirilmiştir.SMA ligasyonu yapılan grup ile kontrol grubu D(-) laktat seviyelerinin istatistiki olarak karşılaştırılması anlamlı olarak bulunmuştur. Diğer iskemi grupları ile kontrol grupları arasında anlamlı farklılık gözlenmemiştir. L(-) laktat serum seviyelerinde ise kontrol gruplarıyla, iskemik model oluşturulan gruplar arasındaki karşılaştırmada istatistiki anlamlılık gözlenmemiştir.Yine kontrol gruplarıyla iskemik gruplarda SGOT ve SGPT istatistiki olarak anlamlı bulunmuş, K+ ise anlamlı bulunmamıştır.Standart klinik kriterlerle; pulse oksimetreye göre yapılan rezeksiyon uzunlukları Wilcoxon testi kullanılarak yapılan değerlendirilmede anlamlı bulunmuştur. Proximal segmentte spesivite ve sensivite distal segmente göre daha yüksek bulunmuştur.
Abstract
The early diagnosis of acute intestinal ischemia and infarctus and the determination of bowel viability border during surgery are always difficult processes for surgeons. In this study; it was investigated in the rabbits in which we performed acute intestinal ischemia, if serum levels of K+, SGOT, SGPT and D(-) lactate have any prognostic value in prediagnostic of intestinal ischemia and standart clinical criteria and pulse oximeter have any significance in the determination of the intestinal viability. In the Department of General Surgery of GATA, we tried to determine the non invasive parameters of acute intestinal ischemia which could be used for early diagnosis of the intestinal ischemia. We made ischemia models in rabbits and compared with control groups. Stood samples were collected from both groups at fifth minute, second and fourth hour and the D (-) lactate and L(-) lactate levels of both groups were determined. At the end of the fourth hour, the serum K, SGOT and SGPT levels were measured. Reexploration was performed at the end of fourth hour and the viable margin of ischemic segment was determined according to standard clinical criteria and pulse oximetric measures. The resected segment was recorded as centimeter and evaluated histopathologically Mann-Withney U test and Wilcoxon test were used for statistical analysis. There was statistical significance between D(-) lactate levels of SMA ligated group and control group. There wasn't any statistical significance between the other ischemia groups and control groups. When L (-) lactate serum levels of both groups were compared, no statistical significance was found in the comparasion of SGOT and SGPT levels pf both groups; there was a statistical significance, but no significance was found for K+ levels. When the segments resected by using pulse oxymeter and by using standard clinical criteria were compared, it was statistically significant. The specifity and sensitivity in the proximal segment was higher than distal segment.
