Febril nötropenik olgularda sefepim+amikasin kombinasyonun etkinliği

Çalışma 8' i kadın 16'sı erkek 24 olgudaki 28 febril nötropenik atakta yapıldı. Olguların yaş ortalamaları 42.71 ± 4.34 (x ± S.E.M) ve yaş dağılımı 20-79 yıl idi. Olguların 15'i akut nonlenfoblastik lösemi, 4'ü akut lenfoblastik lösemi, 3'ü ağır aplastik anemi, 1'i akut bifenotipik lösemi ve 1'i myelodisplastik sendrom idi. Febril atakların %82'sinde santral venöz kateter kullanıldı. Febril ataklar esnasında nötrofil değerleri ortalama 168 ± 40/mm3 (x ± S. E.M) (0-600) olarak saptandı. Bu olgulara sefepim (3x2 g/gün) ve amikasin (15 mg/kg/gün) kombinasyonu ampirik olarak uygulandı. Febril atakların 13'ünde (%46) mikrobiyolojik veya klinik olarak enfeksiyon nedeni saptanmadı. İki febril atakta Metisilin dirençli Staphylococcus aureus ve Aspergillus fumigatus'a bağlı süperenfeksiyon gelişti. Dört olgu sepsis, diğer iki olgu ise kontrol edilemeyen ateş ve kafa içi kanama nedeni ile kaybedildi. Uygulanan antibiyotik kombinasyonuna bağlı WHO grade II'den daha ileri toksisite görülmedi. Bu kombinasyonun uygulandığı febril ataklarda klinik yanıt %57 (16/28) olarak belirlendi. Klinik yanıt sağlanamayan olgularda 72. saat sonunda tedaviye glikopeptid eklenmesi (n=2) ile toplam %64 (18/28) ateş kontrolü sağlandı. Antifungal tedavi ile kontrol edilebilen veya fungal ajan izole edilen febril ataklarda (n=5) değerlendirme dışında bırakıldığında, yanıt % 82 olarak saptandı.
Abstract
Clinical trial was done in 24 neutropenic patients (8 female and 16 ma/e) who had 28 febrile episodes. Their ages ranged between 20 and 79 years (mean ±S.E.M., 42,71 ±4,34). Diseases of these cases were acute nonlymphoblastic leukemia (n:15), acute lym-phoblastic leukemia (n:4), severe aplastic anemia (n:3), acute biphenotyping leukemia (n:1), myelodysplastic syndrome (n:1). Central venous catheter was used in 82 % of episodes. The average neutrophil count was 168 ± 40/mm3 (x + S.E.M.)(0-600) during these episodes. The combination of cefepime (3x2 g/day) and amikacin (15 mg/kg/day) was administered empirically in febrile episodes. Microbiological or clinical cause was not found in 13 (46%) of the febrile episodes. Superinfections due to meticilline resistant Staphylococcus aureus and Aspergillus fumigatus were developed in 2 febrile episodes. Totally six cases died; deaths were due to sepsis in 4 cases and also uncontrolled fever and intracranial hemorrhage in 2 cases. After the administration of the combination therapy, no toxicity was observed over WHO grade II. The clinical success rate with this combination regimen was observed in (%57) 16 out of 28 episodes. By addition of glycopeptide to combination treatment at the end of 72 hours in clinically uncontrolled cases (n=2), fever control was obtained (64%) 18 out of 28 febrile episodes. The success rate of this combination regimen was found 82%, except febrile episodes controlled by using anti-fungal agent or those caused by microbiologically isolated fungal agents (n=5).
