Sistemik lupus eritematozuslu hastalarda akciğer tutulumunun araştırılmasında yüksek rezolusyonlu bilgisayarlı tomografi, pulmoner fonksiyon testleri ve akciğer grafisinin karşılaştırılması

Özet
Sistemik lupus eritematozus (SLE) seyrinde akciğer ve plevra tutulumu sık olarak gözlenir. Bu çalışmada SLE'de akciğer tutulumunun gösterilmesinde yüksek rezolusyonlu bilgisayarlı tomografi (YRBT), pulmoner fonksiyon testleri (PFT) ve P/A akciğer grafisi (Acg) karşılaştırılmıştır. Çalışmaya yaş ortalaması 29,13 ± 8,73 yıl (16-45), ortalama hastalık süresi 33,46 ± 63,27 ay (1-360) olan 8 erkek, 31 kadın SLE'li hasta dahil edilmiştir. Bu hastalarda P/A Acg, PFT (solunum fonksiyon testleri ve karbon monoksit diffüzyon kapasitesi) ve YRBT incelemesi yapılmıştır. Akciğer tutulumunun belirlenmesinde YRBT, P/A Acg'den anlamlı olarak üstün bulunmuştur (p<0,05). PFT ile YRBT arasında istatistiksel anlamlılık bulunmamıştır. Ancak YRBT bulguları esas alındığında PFT'nin akciğer tutulumunu göstermede sensitivitesi % 55,00, spesifitesi ise % 78,95; interstisyel akciğer hastalığını göstermede ise PFT'nin sensitivitesi % 50,00, spesifitesi ise % 78,95 olarak hesaplanmıştır. Sonuç olarak, SLE'li hastalarda akciğer tutulumunun saptanmasında P/A Acg yetersiz kalmaktadır. Her ne kadar PFT ile YRBT arasında istatistiksel fark bulunmasa da, YRBT bulguları baz alındığında PFT'nin sensitivitesinin düşük olması SLE'li hastalarda akciğer tutulumu ve interstisyel akciğer hastalığının gösterilmesinde tüm hastalara YRBT incelemesinin yapılmasının uygun olacağını desteklemektedir.
Abstract
Involvement of the lungs and pleura is common in systemic lupus erythematosus (SLE). In this study, the role of high resolution computed tomography (HRCT), pulmonary function tests (PFTs) and chest radiography (CXR) findings in detection of pleuropul-monary involvement in patients with SLE were evalu¬ated. Pulmonary function tests were contained respiratory function tests with spirometry and carbon monoxide diffusion capacity. 39 patients (8 male, 31 female) were included in this study. The mean age was 29,13 ± 8,73 years (16-45). The mean disease duration was 33,46 ± 63,27 months (1-360). All of the patients had posteroanterior (P/A) CXR, PFTs and HRCT examination. HRCT abnormalities were identi¬fied in 20 patients (51 %), PFTs abnormalities were observed in 15 patients (38 %). P/A chest radiograph was abnormal in only 10 patients (25 %). There was a statistically significance between HRCT and P/A CXR findings in the detection of pleuropulmonary involvement in patients with SLE (p < 0,05). Although there was no statistically significance between PFTs and HRCT findings, when we took the HRCT findings as the base PFTs had 55,0 % sensitivity and 78,95 % specifity to detect pulmonary involvement. P/A CXR was not found to be sufficient for detection of lung involvement in patients with SLE. Although we could¬n't show statistically difference between PFTs and HRCT findings, PFTs were found to have low sensitivity in evaluation of pulmonary involvement in SLE. Thus, we conclude that HRCT examination all of SLE patients would be appropriate to detect lung involvement and interstitial lung disease.
