Özet

Yüz protezleri, çeşitli patolojiler veya travma sonucu oluşan yüz defekti bulunan hastalarda cerrahi sonrası tamamlayıcı rol oynamaktadır. Orta yüz defektlerinin rehabilitasyonu, çene yüz protezleri için daima karmaşık bir konu olmuştur. Orta yüz defektleri kozmetik ve fonksiyonel açıdan eksikliklere neden olmaktadır. Etyolojisi bilinmeyen yıkıcı granulomatoz orta hat granulomlar, defekte neden olan en önemli etkenlerden birisidir ve paranazal sinüs, burun ve damak bölgelerini kapsamaktadır. Bu çalışmamızda, Gata Haydarpaşa Eğitim Hastanesi Diş Servisine bazal hücreli granülom rahatsızlığı ile başvuran 67 yaşında, erkek hastanın olgu sunumu anlatılmıştır. Hasta paranazal sinüsleri, burun, damak, üst dudak, yüzün yumuşak dokularını içeren ciddi orta yüz defektine sahipti. Hasta implant destekli yüz protezi ile rehabilite edildi. İmplant destekli protetik tedavi açık yüzeydeki yumuşak dokuların kapanmasını sağlamıştır. İkincil olarak ise bu tip protezler hastaya psikolojik ve estetik yardım sağlamaktadır.

Abstract

Prosthetic rehabilitation of midfacial defect: A case reportFacial prostheses act as an alternative and supplement for the surgical reconstruction of the patients with facial defects. Rehabilitation of midfacial defects always been a complicated issue for maxillofacial prosthodontists. These midfacial defects lead to functional and cosmetic deficiencies. One of the causes of such defects is midline basal cell granüloma, which is a destructive granulomatous lesion of uncertain etiology, involving the nose, paranasal sinuses and the palate. Presented here is a clinical report of a 67 year-old male patient diagnosed to have lethal midline granuloma, who was referred to the GATA Haydarpasa Training Hospital Dental Service. The patient had a severe midfacial defect involving the nose, the paranasal sinuses, the palate and upper lip and the soft tissues of the face. Patient was rehabilitated implant supported facial prostheses. The implant supported prosthetic rehabilitation was to provide closure of the severe defect to protect the soft tissues from environmental exposure. Secondarily the prosthesis also provided acceptable aesthetics and psychological benefit to the patient.
