Sitomegalovirüs infeksiyonunun prenatal tanısı ve idaresi: Beş olgu

Özet

Amaç: Olası sitomegalovirus infeksiyonu nedeniyle 5 fetus ultrasonografi ve kordosentez kombinasyonu ile incelendi. Materyal ve metod : Bütün hastalara detaylı ultrasonografik inceleme yapıldı ve kordosentez uygulandı. Fetal kan örneklerinde tam kan sayımı, gama-glutamil transpeptidaz (GGT), total IgM ve CMV spesifik IgM araştırması yapıldı. Bulgular: İki olguda maternal primer infeksiyon saptandı. Bunların her ikisi de ultrasonografik olarak normaldi ve bir fetusun infekte olduğu saptandı. Diğer iki olguda tanı anormal ultrasonografik bulgu araştırmasında kondu, ikinci grupta maternal infeksiyon hikayesi yoktu. Bütün pozitif ve negatif tanılar kontrol edildi ve infeksiyon saptanan üç gebelik sonlandırıldı. Sonuç : Ultrasonografi ve kordosentez kombinasyonuyla prenatal sitomegalovirus infeksiyonu tanısının konabileceği sonucuna vardık.

Abstract

Objective: Five fetuses were evaluated with combination of ultrasonography and blood sampling for possible cytomegalovirus infection. Material and method : All cases underwent detailed ultrasonographic evaluation and cordocentesis. Fetal blood samples were subjected whole blood count, gama-glutamil transpeptidase, total IgM and specific IgM measurements. Findings : In two the mother had a documented primary cytomegalovirus infection. Both women had normal ultrasonographic findings and one fetus was found to be infected. In the other two cases fetal cytomegalovirus infection was diagnosed in association with abnormal ultrasonographic findings. There was no history of maternal infection in the latter group. All positive and negative diagnoses were confirmed and none of the three infected fetuses survived. Conclusion : Prenatal diagnosis of fetal cytomegalovirus infection is possible with combination of ultrasonography and blood sampling.
