Özet

Çocuklarda kullanılan geri solumasız anestezi devreleri olan Jackson-Rees modifikasyonu ve Mapleson A devreleri etkinlikleri bakımından karşılaştırıldı. GATA Etik Kurulu'nun iznini takiben, ebeveynlerinden aydınlatılmış yazılı onam alınan, elektif kardiyovasküler sistem dışı cerrahi geçirecek, ASA l-ll grubu 30 infant ve çocuk rastgele iki eşit gruba ayrıldı. Grup l olgular Jackson-Rees pediatrik solunum devresi ile, Grup II olgular Mapleson A devresi ile spontan solutuldu. Tüm olguların anestezi indüksiyonu ve idamesi süresince, sistolik ve diastolik kan basıncı, nabız, solunum sayısı, $SaO_2$ $EtCO_2$ve solunum havasındaki izofluran konsantrasyonları ölçülerek kaydedildi. Grupların hemodinamik ve solunumsal parametrelerinin ortalamaları esas alındığında; izofluran konsantrasyonları arasında istatistiksel olarak anlamlı bir farklılık saptanırken (p 0.05). Sonuç olarak, her iki pediatrik solunum devresinin etkinliği yönünden ve değerlendirilen parametreler açısından önemli bir farklılık tespit edilmedi.

Abstract

The non-rebreathing systems that are used in children, namely Mapleson A and Jackson-Rees modification were compared for the determination of their efficacy. After the approval of Local Ethics Committee and the informed consents of the parents thirty infants and children of ASA l-ll physical status who were scheduled for noncardiovascular elective surgical procedures were randomly divided into two groups. The group I subjects breathed spontaneously by Jackson-Rees pédiatrie breathing system, while group II breathed by Mapleson A system. All the subjects were evaluated during the induction and maintenance of the anesthesia by measuring systolic and diastolic arterial blood pressures and pulse rates, the respiratory rate, $SaO_2$ $EtCO_2$ and the concentration of isoflurane in the breathing air. When the mean values of hemodynamic and respiratory parameters were evaluated in all subjects, pulse rate and isoflurane concentrations were found statistically different (p< 0.05), while there was no difference for the other respiratory and hemodynamic parameters (p> 0.05). Conclusively; no significant difference between the efficacy of these two pédiatrie anesthesia systems was found when the evaluated parameters were compared.
