Özet

Bu çalışmanın amacı, genital prolapsus şiddeti ile üreter akımındaki obstrüksiyon arasındaki ilişkiyi değerlendirmek ve bu bilgiler dahilinde rutin preoperatif intravenöz pyelografi (IVP) ve/veya renal ultrasonografi (USG) gerekliliğinin araştırılmasıdır. Kliniğimizde 1996 Ocak ile 1998 Aralık tarihleri arasında değişik pelvik organ prolapsusu endikasyonları ile opere edilen hastalar ve bunlardan da IVP ve renal USG yapılanlar ayırt edildi. IVP ya da renal USG yapılan hastalarda saptanan hidronefroz unilateral veya bilateral ve hafif, orta, ağır (I-II-III) derecelerde olmak üzere sınıflandırıldı. Çalışmaya dahil edilen 107 hastanın tanıları, 23' ünde sistosel, 10'unda rektosel, 68' inde uterin prolapsus ve 6'sında cuff prolapsusu idi. Bu hastaların yaş ortalamaları 61.3 10.3 (38-84) ve ortalama pariteleri ise 4 ( 1-11) idi. 107 hastanın 19' unda ( % 18) sonografik veya IVP çalışmaları ile değişik derecelerde hidronefroz bulguları mevcut idi. Hastaların 7' sinde hafif, 8' inde orta ve 4' ünde ise ağır derecede hidronefroz saptandı. Hidronefroz en sık olarak % 23.52 oranı ile uterin prolapsusu olan hastalarda görüldü. Cuff prolapsusu olanlarda % 16.6, sistosel olanlarda % 8.69 ve rektosel olanlarda ise hiç hidronefroz görülmedi. Uterin prolapsus şiddeti de arttıkça, buna paralel olarak hidronefroz şiddeti de yükseldi.

Abstract

The aim of this study was to evaluate the relation between severity of genital organ prolapse and ureter flow obstruction and, according to this data, to determine the necessity of routine preoperative intravenous pyelogram and/or renal ultrasound. The patients who had been operated in our clinic between January 1996 and December 1998 because of different pelvic organ prolapse and those of them with preoperative intravenous pyelogram and/or renal ultrasound were identified. According to preoperative intravenous pyelograms and/or renal ultrasounds, hydronephrosis was established as unilateral and bilateral or graded as mild, moderate, severe (I-II-III) degrees. The diagnoses of 107 patients included the study were as follows, 23 cystocele, 10 rectocele, 68 prolapse of uterus and 6 cuff prolapse. The mean age was 61.3 10.3 (38-84) and median parity was 4 (1-11). Hydronephrosis was detected in 19 of 107 (18 %) patients with intravenous pyelogram and/or renal ultrasound. Seven (% 36) patients had mild hydronephrosis, eight (% 42) had moderate hydronephrosis and four (% 21) severe hydronephrosis. Hydronephrosis was mostly seen in patients with uterine prolapse (23.52 %) whereas 16.6 % in patients with cuff prolapse, 8.69 % in patients with cystocele and none in patients with rectocele. The prevalence of hydronephrosis increased with increasing severity of prolapse.
