Özet

Sitokimyasal ve ultrastrüktürel çalışmalarla meme kanser dokusunda ferritin düzeylerinin arttığı gösterilmiştir. Aggresiv tümörler daha fazla transferin reseptör düzeylerine ve proliferatif indekse sahiptirler. Doku ferritin konsantrasyonu transferrin reseptörlerinin indirekt ölçme yöntemi olabilir ve bir proliferasyon indeksi ve bir prognostik indikatör olabilir. Bu çalışmada erken evre meme kanserli 51 hastada tümör dokusu ferritin düzeyi (TDFD)'nin prognostik değeri ve standart prognostik faktörlerle ilişkisi araştırılmıştır. TDFD ile nükleer grad arasında kuvvetli ilişki bulunmuştur. Bununla birlikte TDFD ile yaş, menopozal durum, tümör boyutu, aksiller nodal durum ve östrojen reseptör (ER) arasında istatistiksel anlamlı ilişki saptanmamıştır. Biz TDFD'nin genel sağkalım (GSK) ve hastalıksız sağkalım (HSK) için anlamlı bir prognostik belirleyici olmadığını bulduk. Tek değişkenli analizde, GSK için anlamlı olan prognostik faktörler tümör boyutu (p=0.009), metastatik aksiller lenf nodlarının sayısı (p=0.0008), ER (p=0.04), grad (p=0.03), yaş (p=0.02) ve menopozal durum (p=0.02) idi. HSK için tümör boyutu (p=0.01), aksiller nodal durum (p=0.0001) ve nükleer grad (p=0.02) anlamlı bulunmuştur. Bu sonuçlar TDFD'nin insan meme kanserinde anlamlı bir prognostik faktör olmadığını düşündürmektedir. Bununla birlikte TDFD'nin prognostik değerini kesinleştirmek için daha fazla hasta içeren çalışmalar gereklidir.

Abstract

Ferritin level has been shown to be increased in breast cancer tissue by cytochemical and ultrastructural studies. Aggressive tumors have higher transferrin receptor levels and greater proliferative index. Tissue ferritin concentration may be an indirect method of measuring transferrin receptors and thus might be an index of proliferation and a prognostic indicator. In this study, the prognostic value of the tumor tissue ferritin level (TTFL) and its relationship with the established prognostic factors was investigated in 51 patients with early breast cancer. TTFL was significantly associated with nuclear grade. However, there were no statistically significant associations between TTFL and age, menopausal status, tumor size, axillary nodal status, estrogen receptor (ER) status. We found that TTFL was not a significant prognostic indicator for overall survival (OS) and disease-free survival (DFS). In univariate analysis, tumor size (p=0.009), the number of axillary metastatic nodes (p=0.0008), ER (p=0.04), grade (p=0.03), age (p=0.02) and menopausal status (p=0.02) were significantly prognostic for overall survival. For DFS, tumor size (p=0.01), axillary nodal status (p=0.0001), and nuclear grade (p=0.02) had prognostic significance. These results suggest that TTFL is not a significant prognostic factor in human breast cancer but further studies are needed to verify the prognostic value of TTFL in a larger patient population.
