Özet

Nonfonksiyone adrenal korteks karsinomu nadir görülür (1). Klinik olarak asemptomatik seyreder. Her yaşta görülebilir ve görülme sıklığı 1-2/milyon nüfus/ yılda'dır (2, 3). 25 yaşında bayan hasta, iki yıllık evli. Infertilité ve sağ üreter taşı nedeniyle müracaat eden hasta idrar renginde koyulaşma, kilo kaybı, abdominal huzursuzluk ve bilateral tomber bölge ağrısı şikayetleri ile müracaat etti. Direk grafide sağ üreter alt uçta opasite, IVP'de sağ üreterohidronefroz ve sağ parapelvik kist, USG'de sağ böbrekte hidronefroz ve sol adrenal gland lojunda kitle saptanan, BT'de sol adrenal gland lojunda sol surrénale ait olduğu düşünülen 9x8x7 cm. boyutlarında heterojen, hiperplazik, içinde kalsifikasyonlar bulunan, nispeten iyi sınırlı kitle lezyonu saptandı. Hastanın endokrin fonksiyonları normal sınırlarda idi. Sağ üreter taşını medikal tedavi sonrası spontan olarak düşürdü. Eksplore edilen hastadan kitle eksize edildi ve histopatolojik muayene sonucunda adrenal korteks karsinomu saptandı. Adrenal korteks karsinomunda 3 yıllık sürvinin % 37 olması (1) nedeniyle de hastanın kocasına çocuk yapmamaları gerektiği uygun bir ifade ile izah edildi. Bu çalışmada sağ üreter taşı ve infertilité nedeniyle évalue edilirken sol adrenal kitle saptanıp, cerrahi olarak tedavi edilen bir olguyu, büyüklük olarak nadir görülmesi ve kötü prognoza sahip olması nedeniyle sunduk ve literatür eşliğinde tartıştık.

Abstract

Nonfunctioning adrenal cortical carcinoma is an uncommon malignancy, with an incedence of 1-2cases/million population/year. It may be encountered in every age. Approximately 50 % of adrenal cortical carcinomas are functioning with semptoms related to excessive adrenal cortical steroid production. The remaining 50 % are nonfunctioning tumours and these patients present with nonspesific symptoms such as abdominal pain, abdominal mass, fatique and weight loss. A 25-year-old woman married for two years was applied to our outpatient clinic with infertility, hematuria, weight loss, abdominal discomfort and bilateral flank pain. Physical examination revealed blood pressure of 118/72 mmHg., pulse rate of 76 beats/ min., bilateral costovertebral angle tenderness. The right kidney was not palpable, but a mass palpated in the region of the left kidney. Flat graphy revealed a stone in the lower tip of the right ureter. IVP showed the right ureterohydronephrosis and right renal cali-ectasia. The right ureterohydronephrosis and a mass in the anatomical location of the left adrenal gland were detected with abdominal ultrasonography. CT revealed a mass shifting the spleen to the left and compressing the left kidney superficially, with 9x8x7 cm. size, heterogenous, hyperplastic, relatively well limited and having calcifications inside. The endocrin functions were normal. The patient was explored and the mass was excised completely. Histopathological examination revealed the adrenal cortical carcinoma. The patient passed the right ureter stone with medical treatment spontaneously. We report this case, which was detected incidentally during evaluating of right ureter stone and infertility, because of its rarity and poor prognosis.
