Özet

Tüberküloz mücadelesinde en önemli basamaklardan bir erken tanı ve tedavidir. Türk Silahlı Kuvvetleri'nde bulunan aktif tüberkülozluların tanı ve tedavisindeki gecikmeler hastalığın daha fazla yayılmasına neden olmaktadır. Tüberküloz tanı ve tedavisindeki gecikmeyi tanımlamak amacıyla bu çalışma planlandı. Hastanemize sevk ile gelen 52 akciğer tüberkülozu tanısı konmuş er çalışmaya alındı. Olgularımızın yaş ortalaması 21.85±2.56. askerlik süresi 9.91±5.87 ay olup, olguların % 60'ı ilkokul mezunu idi. Başlıca yakınmalar halsizlik (% 65), öksürük (% 62), gece terlemesi (% 53) ve zayıflama (% 48) olarak saptandı. Olguların yakınmaları başladıktan sonra % 63'ünün hemen viziteye çıktığı, % 37'sinin ise viziteye çıkamadığı, çıkamayanların % 53'ünün kendi dışında nedenlerle, % 47'sinin ise kendi ihmali nedeni ile viziteye çıkmadıkları öğrenildi. Olgularımızın % 55.57 şikayetleri başladıktan sonra 15 gün içinde, % 44.5'i ise 15 günden daha geç sürede hastanemize ulaşmıştır. Şikayetleri başladıktan sonra 15 gün içerisinde hastanemize gelen olguların % 67'sine ve 15 günden daha geç gelenlerin ise % 70'ine ilk 5 gün içerisinde antitüberküloz tedavi başlanmıştır. Sonuç olarak; olgularımızın yaklaşık yarısı yakınmaları başladıktan 15 günden daha uzun süre geçtikten sonra hastaneye ulaşmış ve bunların çoğuna ilk 5 gün içerisinde antitüberküloz tedavi başlanmıştır. Hastalığın semptomlarının oluşmasına kadar geçen zaman da göz önüne alındığında bu gecikme daha da önemli olmaktadır.

Abstract

Early diagnosis and treatment is a mainstone in tuberculous control. Delay in diagnosis and treatment of tuberculosis may cause dissemination of the disease. This study was designed to assess the time to starting antituberculous therapy in Turkish Armed Forces. Fifty-two soldiers with pulmonary tuberculosis was included in the study. In the study group, mean age was 21,85±2.56 years and duty period was 9.91 ±5,87 months. Education level was primary school in 60% of the patients. Main symptoms were malaise (65%), cough (62%), night sweat (53%) and weight loss (48%). Sixty-three percent of patients admitted that they had visited to infirmary as soon as their complaints had been started and other37% of the patients couldnt visit the doctor. In latter group, causes of delay in administration to doctor after starting the symptoms were due to factors unrelated to the patients themselves. Fifty-five point five percent of the cases was admitted our hospital into 15 days after starting their symptoms. Other 44.5% of the cases delayed our hospital more than 15 days. Antituberculous therapy was started into 5 days after accepted to the hospital in 67% of the cases who were refereed to our hospital into 15 days and in those of 70% who were refereed later 15 days after symptoms started. Conclusions: Approximately half of the cases were administered the chest disease hospital more than 15 days afterheir symptoms had started. Most of these patients received antituberculous therapy into 5 days after they had accepted to the hospital of it is considered that the time period until symptoms appears delay of treatment became more important.
