Kemoterapötik ajanların bağırsak florası ve bakteriyel translokasyona etkileri

Özet
Çalışmamızda çocuklarda sık kullanılan kemoterapötik ajanlardan Methotreksat (MTX), Sitarabin (ARA-C) ve Siklofosfamid (CY)'in bağırsak florası ve bakteriyel translokasyona olan etkisini sıçanlarda araştırmayı amaçladık. Çalışma onar adet sıçandan oluşan dört grupta yürütüldü. Birinci grupta 2.5mg/kg/gün methotreksat, ikinci grupta 7mg/kg/gün sitarabin, üçüncü grupta ise 7mg/kg/gün siklofosfamid günde bir kez üç gün süreyle sıçanlara suprakapsüler bölgeden subkutan verildi. Beşinci gün sakrifiye edilen sıçanların mezenter lenf nodu, kan, dalak, akciğer ve karaciğerinden kültür örnekleri alındı. Barsak florasına etkilerini değerlendirmek için çekotomi yapılarak çekal sürüntü kültürü alındı. Çalışmada metotreksat bakteriyel translokasyona en çok yol açan kemoterapik ajan olup siklofosfamid ve sitarabin ise daha az oranla bakteriyel translokasyona neden olmuşlardır. Translokasyona en çok neden olan organizma Escherichia coli bulundu. Çalışma gruplarında çekal sürüntü kültürlerinde üreyen floraya hakim organizmalar E.coli ve Enterococcus faecalis D bulundu. Sonuç olarak immun sistemi zayıf maligniteli hastalarda kemoterapötik ajanların bakteriyel translokasyon sonucu multiorgan yetmezliğine kadar götüren septik tablolara yol açabilecekleri düşünülmelidir.
Abstract
In our study, we aimed at investigating the effects of methotrexate, cytarabine and cyclophosphamide, agents often used on children to bowel flora and bacterial translocation on rats. The study was carried out on four groups composed of ten rats. 2.5mg/kg methotrexate in the first group, 7mg/kg cytarabine in the second one and 7mg/kg cyclophosphamide were administrated rats subcutaneously from supracapsular part once a day for three days. Culture specimens were obtained from the mesenteric lenf nodul, blood, lung and livers of rats sacrificied on the 5th day. Caecum swamp culture was obtained to evaluate their effects to bowel flora by performing caecotomy. In our study, methotrexate is the agent causing the most bacterial translocation but cyclophosphamide and cytarabine caused less. Escherichia coli was the organism causing the most translocation. Among study groups E.coli and Enterococcus faecalis were found to be organisms dominating the flora multiplying in caecum swamp culture. As a result, it must be taken into consideration that chemotherapic agents may cause septic tables including multiorgan deficiency on patients with malignity whose immune system is weak.
