Akut faz haline örnek olarak seçilen miyokard infarktüsünde serum total ve serbest PSA düzeylerinin araştırılması

Özet
Prostat Spesifik Antijen (PSA), prostat sekretuar epitelinden salgılanan 33 kDa mol ağırlığında tek zincirli bir glikoproteindir. PSA'nın serum proteinleri ile kompleks oluşturduğuna işaret eden yeni çalışmaların ışığında, çalışmamızda akut faz reaktanlarının serum total PSA (TPSA) ve serbest PSA (FPSA) konsantrasyonlarına olası etkileri araştırıldı. Bu amaçla koroner yoğun bakım ünitesine başvuran akut miyokard infarktüslü 59 erkek hasta ve 17 sağlıklı yetişkin erkek incelemeye alındı. MI geçiren hastaların 15'i 1 ay sonra tekrar çağrılarak MI kontrol grubu oluşturuldu. Serum ve plazma örneklerinde TPSA, FPSA, Fibrinojen ve C-Reaktif protein (CRP) düzeyleri ile F/T PSA oranı saptandı. TPSA düzeyleri üç grupta istatistiksel olarak anlamlı bir fark göstermedi. Buna karşılık gerek MI (n=59) gerekse MI kontrol grubunda (n=15) FPSA düzeyi ve F/T PSA oranında sağlıklı kontrollere (n=17) kıyasla istatistiksel olarak anlamlı düşüş saptandı. MI grubu (n=15) FPSA düzeyleri ve F/T PSA oranları beklenildiği üzere fibrinojen ve CRP düzeyleri normale dönmüş olan MI-kontrol (n015) grubuna kıyasla keza anlamlı derecede düşüktü. Çalışmamızdan elde ettiğimiz verilere göre miyokard infarktüsü dahil çeşitli akut hastalık durumları, serum FPSA düzeyi ile F/T PSA oranını etkilemektedir. Daha geniş kapsamlı ve homojen gruplarla yapılacak çalışmalar daha güçlü istatistiksel bilgiler sağlayabilecek ve prostat kanseri teşhis ve takibinde serum PSA ölçümlerinin klinik katkısını yönlendirebilecektir.
Abstract
Prostate Specific Antigen (PSA) is a 33 kDa single chain glycoprotein that is produced in the prostatic secretory epithelium. In light of recent studies showing that PSA complexes with certain serum proteins, we studied the effects of acute phase reactants on serum concentrations of total and free PSA. 59 men admitted to coronary care unit with myocardial infarction were investigated as well as healthy controls (n=17). Patients with Ml (n=15) were also included to study after 1 month of period as Ml-control group. Serum TPSA, FPSA and CRP levels, plasma fibrinogen levels and F/T PSA rates were determined. The differences between these three groups in TPSA levels were not statistically significant. However, a statistically significant decrease was observed in FPSA levels and F/T PSA rates in both Ml (n=59) and Ml control (n=15) groups compared to healthy controls (n=17). In Ml group (n=15) FPSA levels and F/T PSA rates were also significantly lower compared to Ml-controls (n=15) whose have normal fibrinogen and CRP concentrations as expected after one month of period. The data obtained from the present study indicates that various acute conditions including myocardial infarction may effect the serum concentration of FPSA and F/T PSA rates. Additional studies with a larger and more homogeneous group of patients will provide stronger statistical conclusions and have a clinically significant impact on the use of PSA for detecting prostate cancer.
