Özet

Amaç: Splenektomi ve splenik ototransplantasyon yapılmış ratlarda, sepsiste oluşan hematolojik, biokimyasal, histopatolojik değişiklikler ve mortalite oranları araştırıldı. Yöntem: Çalışmada 90 adet rat laparatomi, splenek¬tomi, splenik ototransplantasyon grubu olmak üzere üçe ayrıldı. Postoperatif 20 haftalık takip periodun-dan sonra her gruptan 20 rata 2 ml S.pneumonia susu (1x107 /mi) 2 ml intraperitoneal olarak verildi. Sepsis oluşturulduktan 24 saat sonra her gruptan sepsis oluşturulan ve oluşturulmayan 10 ar adet rata relaparatomi yapılarak kan ve doku örnekleri alındı. Sepsis oluşturulan diğer 10 rat da mortalite oranlarını tesbit amacıyla takip edildi. Bulgular: Ototransplantasyon grubundaki implant-ların hepsi gelişirken, splenektomi grubunda diğer gruplara göre artan trombosit sayısının, sepsis oluş¬tuğunda anlamlı olarak azaldığı (p<0.001) ve lökositin anlamlı olarak arttığı gözlenmiştir (p<0.001). Gruplar arasında total biluribin, AST.LDH, kreatinin değerleri benzer bulunurken, splenektomi-sepsis grubunda albumin anlamlı düşük (p<0.001), TNF-a ise anlamlı olarak yüksek (p<0.001) bulun¬muştur. Ototransplantasyon grubunda sepsisin neden olduğu KC, AC, mezenter, kemik iliği ve cilt hasarının splenektomi grubuna göre daha az olduğu görülmüştür. 5. gün mortalite oranları laparatomi grubunda % 50, splenektomi grubunda % 80 ve oto¬transplantasyon grubunda % 60 olarak bulunmuştur. Sonuç: Splenik implantların 20 hafta sonunda nor¬mal dalak yapısına benzer şekilde geliştiği ve sepsis oluşturulduktan sonra da hematolojik, biokimyasal ve histopatolojik olarak olumlu etkilerinin olduğu, mortalite oranını azalttığı görülmüştür.

Abstract

The effects of sepsis on hematological, biochemical, histopathological parameters and mortality rates in splenectomised and spleen autotransplanted rats were investigated. In this study 90 rats were divided into three groups namely; laparotomy, splenectomy and autotransplan¬tation. Postoperatively, after 20 weeks follow-up peri¬od 20 rats of each groups were given S.pneumonia toxin into the peritoneum. 24 hr after sepsis manage¬ment 20 rats were performed relaparotomy and blood and tissue samples were taken out. 10 of them were in sepsis group and the other 10 weren't. 10 rats which were subjected to sepsis were followed for mortality rates. Platelet levels that were increased after splenectomy were decreased with sepsis management. However leukocyte levels were highest in splenectomy group (p<0.001). There was no difference about total biliru¬bin, AST, LDH and creatinin levels. But the albumin levels were lower and the TNF-a levels were higher in the splenectomy-sepsis group (p<0.001). Hepatic, pulmonary, bone marrow and cutanea! lesions which is clearly seen in sepsis were lower in autotransplan¬tation group than in splenectomy group. Fifth day mortality rates were %50, % 80 and % 60 for laparatomy, splenectomy and autotransplantation groups, respectively. Spleen autotransplantation revealed beneficial effects on hematological, histopathological and bio-chemistrial parameters thus mortality rate was decreased.
