Tiroid nodüllerinde preoperatif sitolojik ve postoperatif histopatolojik sonuçların karşılaştırılması

Özet
Tıroid bezi patolojilerinin önemli bir bölümü nodüler gelişim ve bu nodüllerde kanserleşme eğilimi oluşturmaktadır. Soliter tiroid nodüllerinde %15-25'e varan maügnite olasılığı söz konusu olduğundan, operatif tedavinin planlanabilmesi için preoperatif sitolojik tanı büyük önem taşımaktadır. Son 20 yıldır tiroid nodüllerinin tanısında ince iğne aspirasyon biyopsisi (IIAB), çeşitli radyonüklid ajanlar ve duyarlı ultrosonografi yaygın olarak kullanılmaktadır. Bu çalışmada, nodüler tiroid patolojisi nedeniyle preoperatif IIAB ve sonrasında operasyon uygulanan hastalarda preoperatif sitoloji ile postoperatifhistopatolojik sonuçların karşılaştırılması amaçlandı. Çalışmaya soliter tiroid nodülü, dominant soğuk nodülü olan multinodüler guatr ve nodul ayırt edilemese de tiroid dokusunun irregüler yapıda olduğu olgular nedeni ile öpere edilen ve ameliyat öncesi IIAB uygulanan 154 olgu dahil edildi. Hastaların hepsine daha önceden tiroid sintigrafisi, ultrasonogram ve tiroid fonksiyon testleri yapıldı. IİAB yapılan 154 olgudan 126 olgu benign (%81.8), 24 olgu şüpheli (%15.6), 4 olgu malign (% 2.6) olarak rapor edildi. Ameliyat esnasında makroskopik olarak 82 olguda (%53.2) multinodular, 49 olguda (%31.9) soliter, 23 olguda (%14.9) kistik nodüler gelişme saptandı. Histopatolojik inceleme sonucunda ise 133 olguda (%86.4) benign guatr, 14 olguda (%9.1) benign tümör (adenom) ve 7 olguda (%4.5) malign tümör (kanser) saptandı. IİAB ile saptanamayan 3 malign olgunun hiçbiri şüpheli grupta değildi. llAB'nin sensitivitesi (duyarlılık) %46, şpesivitesi (özgünlük) %93, doğruluk oranı %85, negatif prediktif değer (eksi yorum gücü) %88, pozitif prediktif değer (artı yorum gücü) %61, yalancı negatif oran %6.3, yalancı pozitif oran %53 olarak bulundu.Sonuç olarak, llAB'nin cerrahi hasta seçiminde ve tiroid karsinomalarının ameliyat öncesi tanısında kullanılabilecek ucuz, güvenilir ve komplikasyonsuz bir tanı aracı ve tiroid ameliyatlarının preoperatif planlanmasında değerli bir yöntem olduğu kanısına varıldı.
Abstract
Nodular changes in high proportion of thyroid pathologies and malignant inclination in some of these nodules may be present. Since 15-25% proba¬bility of malignancy may be present especially in solitary thyroid nodules, preoperative cytological diagnosis is highly important for planning of operative treat¬ment. Fine needle biopsy (FNB), some radionuclide substances, and sensitive ultrasonography in diagnosis of thyroid nodules have been widely used for the last two decades. In the study, comparison of preoperative cytological and postoperative histopathological findings in patients with thyroid nodules who had FNB preoperatively and operative intervention thereafter was aimed. One hundred and fiftyfour patients who had FNB preoperatively and operative management for their solitary thyroid nodule, or multinodular thyroid with a dominant cold nodule, or irregular thyroid without a nodule were included in the study. Before these procedures, all patients had radionuclide study, thyroid ultrasonography, and thyroid function tests. Preoperatively, 126 of cases (81.8%) were diagnosed as benign, 24 cases (15.6%) as suspicious and 4 cases (2.6%) as malign in cytology. In 82 cases (53.2%) multinodular, 49 cases (31.9%) solitary and in, 23 cases (14.9%) tys-ticnodular changes were detected during operation. Histopathologically, 133 cases (86.4%) were diagnosed as benign goitre, 14 cases (9.1%) as benign tumour (adenoma), and 7 cases (4.5%) as malignant tumour. Three malignancies, which were not detected by FNB, were not in suspicious group. Sensitivity, specificity, diagnostic accuracy, negative predictive value, positive predictive value, false negative rate and false positive rate of FNB were found as 46%, 93%, 85%, 88%, 61%, 6.3%, and 53%, respectively. As a result, FNB was found to be a useful, less complicated, cheap and reliable diagnostic tool, which may be used in selection cases for operative treatment and in preoperative diagnosis of thyroid malignancies and it was also, regarded as a valuable method in planning of operative intervention.
