Özet

Epilepsi aslında klinik bir tanıdır. Epilepsi tanısına; EEG, nörolojik muayene ve radyolojik yöntemler yardımcı olsa da, tanı temelde anamneze dayanır. Bu nedenle epilepsi tanısını koymada güçlüklerle karşılaşılabilir ve yanlış tanılar konulabilir, gereksiz yere antiepileptik ilaç verilebilir. Bu çalışmada, kesin tanı koymadaki güçlükler vurgulanmıştır. Bu çalışmada, 1997 yılında GATA Nöroloji kliniğine bayılma nedeniyle yatan hastalar incelendi. Tüm hastaların detaylı anamnezi alındı, hayatının herhangi bir döneminde epilepsi tanısı konularak antiepileptik ilaç (AEİ) kullanıp kullanmadığı sorgulandı, Hastaların tamamının interiktal EEG'leri çekildi ve bu hastalar belirli bir süre yatırılarak (ortalama 12.6 gün) bayılma nöbetleri gözlendi. Çalışmamıza yaş ortalaması 21.5 yıl olan, 4'ü kadın, 178'i erkek toplam 182 hasta alındı. Bu hastalardan 104'ünün nöbeti gözlenirken, 78 hastanın bu süre içinde nöbeti olmadı. Nöbeti gözlenen 104 hastanın 69'u (%66) epiieptik, 357 (%34) de non-epileptik nöbet geçirdi. Hem AEl kullanan hem de nöbet geçiren 74 hasta tespit edildi. Bunların 57'si (%77) epiieptik, 17'si (%23) de non-epileptik nöbet geçirdiler. Bir başka ifadeyle epiieptik nöbeti olan hastaların %82.6'sınm, non-epileptik nöbet geçirenlerin de %48.5'inin AEl kullanım öyküsü vardı. Epilepsi tanısı temelde anamneze ve EEG bulguları¬na dayandığından hastalara yanlış tanılar konulabildiği, bizzat nöbetin kendisini görmek ya da iktal EEG ve videomonitörizasyon kaydı ile daha doğru tanı konulabileceği sonucuna varılmıştır.

Abstract

Epilepsy is essentially a clinic diagnosis. Although investigation such as neurologic examination and EEG may provide, the diagnosis is usually made on the basis of the clinical history. In this study we found that there are many practical problems for the diag¬nosis of epilepsy, these patients have been misdiagnosed and given antiepileptic medication unnecessarily. In this study, the patients, admitted to Gülhane Military Medical Academy, Department of Neurology, in 1997 for the complaint of seizures were examined. All detailed clinical histories were taken and they were particularly questioned about the use of antiepileptic drug (AED) in their life time. Their inte-rictal EEGs were taken. They were hospitalized for a while (mean 12.6 days) in order to seizure observa¬tion. We examined 4 women, 178 men, totally 182 patients whose mean age 21.5 years. During this observation 104 of them had seizures. In this group 69 (%66) patients had epileptic, while other 35 (%34) had non-epileptic seizures. Fifty-seven (%77) epileptic and 17 (%23) non-epileptic totally 74 patients had both seizures and clinical history of use of AEDs. In other words %82.6 epileptic, %48.5 non-epileptic patients had clinical history of use of AEDs. Since the diagnosis of epilepsy is usually made on the basis of the clinical history there should be misdiagnosed patients and the observation of the seizures or ictal EEG and long-term video-EEG monitoring is the best way for the diagnosis of epilepsy.
