Diabetik ayak ülserlerinin tam temas alçısı ile tedavisi ve sonuçlarımız

Özet
Su çalışmamızın amacı diabetik hastalarda gelişen ayak ülserlerinin tedavisinde pratik bir yöntem olan tam temas alçısı uygulamamızın sonuçlarını sunmak ve literatür bilgileri altında değerlendirmesini yapmaktır. Kasım 1996-Ocak 1999 yılları arasında diabetik ayak ülseri nedeniyle takip edilen ve tam temas alçısı uygulanan 10 vaka (3 tanesi Wagner tip I, 7 tanesi tip II) çalışma kapsamına alındı. Glisemik kontrol sağlandıktan sonra yara bakımını takiben her hastaya aynı yöntemle tam temas alçısı yapıldı. Alçı uygulandıktan sonra 12 saat süre ile ayağa elevasyon uygulandı. Daha sonra rocker tabanlı alçı terliği ile hastalar mobilize edildi. Yara yerlerinden kapak açılarak pansuman takibi yapıldı. Hastalarımıza ortalama 6 hafta süreyle tam temas alçısı uygulandı. Hastalarımız ortalama 24 ay süreyle takip edildi. Bir hastamızda uyum sorunu yaşandı ve tam temas alçısını kullanamadı. Hiçbir hastamızda alçı uygulaması sırasında ayakta akıntı ve kötü koku gibi komp-likasyonlarla karşılaşılmadı. Sübjektif ve fonksiyonel değerlendirmede, 6 vakada mükemmel, 2 vakada orta ve 2 vakada kötü sonuç elde edildi. Hiçbir hastamızda rekürrent ülser gözlenmedi. Sonuç olarak tam temas alçısı, ön ve orta ayak bölgesinde Wagner tip I ve II ülseri olan DM'lu hastaların tedavisinde iyileşmeyi hızlandırması, uygulama kolaylığı, hasta¬ların poliklinikte takibinin yapılabilmesi, pansuman sayısının azaltılması ve iş gücü ve maliyet kazancı nedeniyle seçilebilecek güvenilir yöntemlerden birisidir.
Abstract
The aim of this study is to report our results of total contact casting which is a practical and reliable method in the treatment of diabetic foot ulcers and to evaluate these in the light of the literature. Ten cases (3 cases Wagner grade I and 7 cases grade II) that were being followed for diabetic foot ulcer and to whom total contact casting was applied between November 1996 and January 1999 were included in this study. In all cases, first blood glucose level was controlled then wound care was taken and total contact casting was applied by using the same method in each case. Foot was elevated and immobilized for 12 hours after casting was applied. After that, the patients were mobilized by using rocker sole casting slippers. Window was opened on the ulcer site to do wound dressing. We applied total contact casting to our patients for 6 weeks on average. The patients were followed up for 24 months on average. One patient had a problem of adaptation so total contact casting was not used. In none of the patients complications like bad smell or purulent material were encountered. In the subjective and functional evaluation, 6 perfect, 2 good, and 2 bad results were obtained. Recurrence was not seen in any of the patients. As a result, total contact casting is a reliable, practical and costeffective method that can be chosen in the treatment of patients with diabetes mellitus who have Wagner grade I and grade II ulcer since it accelerates the healing, decreases the number of dressings, is easy to apply, decreases the loss of workforce.
