Işınlamanın ve kısa süreli depolamanın aferez ürünü trombosit fonksiyonları üzerine etkisi

Özet

Transfüzyona bağlı graft versus host hastalığı (TB-GVHH) seyrek gözlenen ciddi bir transfüzyon komplikasyonudur. TB-GVHH'nın etkin bir şekilde önlenmesinde kullanılan tek yöntem, transfüzyon öncesi hücresel kan ürünlerinin ışınlanmasıdır. Bu çalışmada, ışınlamanın ve depolamanın 16 ayrı sağlıklı donörden elde edilen aferez ürünü trombositlerin fonksiyonları üzerine olan in vitro etkilerini araştırdık. Işınlama öncesi-sonrasında ve aferez ürünü trombositlerin depolanması sırasında 0., 1,, 3. ve 5. günlerde adenozin difosfat (ADP), kollajen ile uyarılmış in vitro trombosit agregasyonu ve kollajen ile uyarılmış adenozin trifosfat (ATP) salmımı lumidual agregometre ile çalışıldı. Işınlamanın ADP ve kollajen'e bağlı in vitro trombosit agregasyonu ve ATP salınımı üzerine azaltıcı etkisi gözlenmedi. Depolama süresi boyunca ADP ve kollajen'e bağlı in vitro trombosit agregasyonunda önemli azalma gözlenmezken, ATP salınımında 0. güne göre (1.64 ± 0.16 nmol), 1. günde (0.92 ±0.17 nmol) ve 3. günde (0.36 ± 0.09 nmol) belirgin azalma gözlendi ve 5. günde hiç ATP salınımı gözlenmedi. Sonuç olarak aferez ürünü trombositlerin ışınlanması, klinik olarak trombosit kalitesinde önemli bir değişikliğe sebep olmamaktadır. Aynı zamanda aferez ürünü trombositlerin hazırlanması ve sonrasında in vitro fonksiyonları değişmeksizin en az 5 gün süre ile depolanması mümkündür. Ancak bu trombositlerin depolanması sırasında trombositlerden ATP salınımında değişiklik olabilmektedir.

Abstract

Transfusion associated graft-versus-host (TA-GVHD) disease is a rare but serious adverse effect of blood transfusion. Irradiation of the blood components before transfusion is an only method used for the prevention of TA-GVHD. Apheresis thrombocytes is usually storage to use for the emergency needing. In this study, we investigated the effect of in vitro thrombocyte activation of irradiation and storage of apheresis thrombocytes in different 16 healthy volunteer donors. Adenosine triphosphate (ATP) release, stimulated with collagen and also in vitro thrombocyte aggregation, stimulated with adenosine diphosphate (ADP), collagen were measured on a lumidual aggregometer before and after irradiation, and during the storage of apheresis thrombocytes on days 0., 1., 3. And 5. The irradiation had no reducing effect ADP-and collagen-induced in vitro thrombocyte aggregation and ATP release. With the increasing time of storage the ADP-and collagen-induced in vitro thrombocyte aggregation was not significantly diminished, but contrary ATP release is significantly diminished on day 1 (0.92 ±0.17 nmol) and day 3 (0.36 ± 0.09 nmol) according to day 0 (1.64 ± 0.16 nmol), respectively (p<0.05 ve p<0.0001, and no ATP release on day 5. In conclusion, irradiation of apheresis thrombocytes does not result in a clinically significant, demonstrable adverse effect on thrombocyte quality. Also, it is possible to apheresis thrombocytes on the day of collection and subsequently store them for at least 5 days while maintaining in vitro function. But these data indicate that storage of apheresis thrombocytes may change of ATP release on thrombocytes.
