Çocuk ve ergenlerde dikkat eksikliği hiperaktivite bozukluğunun alt tipleri ile sürekli performans testi değişkenleri arasındaki ilişki

Özet
Amaç: Bu çalışmada DSM-IV ölçütlerine göre dikkat eksikliği hiperaktivite bozukluğu (DEHB) alt grupları ile Sürekli Performans Testi (SPT) değişkenleri arasındaki ilişki araştırılmıştır, Yöntem: 7-16 yaşları arasındaki DEHB tanısı konan 53 çocuk ve ergen araştırmaya alındı. Annelerine DSM-IV'e dayalı Yıkıcı Davranış Bozukluklarını Değerlendirme Ölçeği verildi. Çocuk ve ergenlere ise SPT uygulandı.Bulgular: DEHB kombine tip %56.6 (n=30), dikkatsizliğin baskın olduğu tip %20.8 (n=11) ve hiperaktivite - impulsivitenin baskın olduğu tip %22.6 (n=12) olarak saptandı. Komorbid tanılar sıklık sırasına göre; karşı gelme bozukluğu %56.6 (n-30), davranım bozukluğu % 15.1 (n=8), öğrenme bozukluğu %7.5 (n-4) ve depresif bozukluk %3.8 (n=2) olarak sıralandı. Yaş ile hedef uyaranı kaçırma (r=-.53, p<.01) ve yaş ile yanıt oluşturma süresi arasında anlamlı derecede negatif korelasyon bulundu (r=-, 47, p<.01). SPT değişkenlerinden yalnızca hedef uyaranı kaçırma ile Yıkıcı Davranış Bozukluklarını Değerlendirme Ölçeğinin dikkatsizlik faktörü arasında anlamlı korelasyon bulundu (r=.29, p<.05). Tartışma: Hedef uyaranı kaçırma ile dikkatsizlik ilişkili bulunurken, basılmayacak yerde basma ile impulsivite arasında ilişki saptanmadı. SPT'nin tanı koyma aracı olarak değil, ancak DEHB'nda tanıyı destekleyici bir test olarak kullanılabileceği düşünüldü.
Abstract
Objective: This study was investigated the relationship between the Continuous Performance Test (CPT) variables and subgroups of attention deficit hyperactivity disorder (ADHD) according to DSM-IV. Method: The subjects were 53 children and adolescents between the ages of 7-16 years. Mothers were asked to complete the Child and Adolescent Behavior Rating Scale which is a diagnostic measure based on DSM-IV. The children and adolescents had performed CPT. Results: The combined type of ADHD was diagnosed in 56.6% (n=30) of the subjects, while 20.8% (n=11) had inattentive type and 22.6% (n=12) hyperactive-impulsive type. There were concurrently 30 (56.6%) children with oppositional defiant disorder, 8 (15.1%) children with conduct disorder, 4 (7.5%) children with learning disabilities, and 2 (3.8%) children with depressive disorder respectively. A significant negative relationship was found between omission errors and their ages (r=-,53, p<.01) and between reaction time and their ages (r=-, 47, p<.01). However, the significant correlation just occurred between omission error and the inattentive factor of the Child and Adolescent Behavior Rating Scale (r=,29, p<.05).Conclusion: As expected, omission errors correlated with inattentiveness, but surprisingly, comission error did not correlate with impulsivity. We suggest that CPT is not a diagnostic tool, but it is a supportive measure for the clinical diagnosis in ADHD.
