Özet
Amaç: Bu çalışma mide karsinomu tanısı alan olgularda, tümör çevresi mide mukozasında neoplastik olmayan prekanseröz lezyonları araştırmak amacıyla yapıldı. Yöntem: Mide karsinomu tanısı alan 38 olguda, tümör çevresi mide mukozasında kronik atrofik gastrit, intestinal metaplazi, epitelial displazi gibi neoplastik olmayan prekanseröz lezyonlar arştırıldı. Bulgular: Olgulardan 24 tanesinde (% 63) intestinal tip, 14 tanesinde (% 37) de infiltran tipte mide karsinomu tesbit edildi. Olgulardan tamamında gastrit saptanırken, bunlardan 18 tanesinde (% 47) kronik atrofik gastrit, 8 tanesinde (% 21) Helikobakter pilori (HP) gastriti, 14 tanesinde (% 37) akut ya da kronik nonspesifik gastrit tablosu mevcuttu. İntestinal metaplazi 19 olguda (% 50), displazi 10 olguda (% 26) tesbit edilirken, 8 olguda (% 21) da rejenerasyon mevcuttu. Sonuç: Sonuç olarak, prekanseröz lezyonların tanınmasının, mide kanseri kontrolünün yanında erken tanı ve tedavisinde yararlı olacağı kanısına varıldı.
Abstract
Objective: The aim of this study is to evalute nonneoplastic precancerous histopathologic changes in gastric mucosa of peripheral tumor with gastric carcinomas. Methods: We investigated non neoplastic precancerous lesions such as chronic atrophic gastritis, intestinal metaplasia and epithelial dysplasia in the gastric mucosa of around of tumoral tissues in 38 cases of gastric carcinomas. Results: 24 of the cases (63% ) were intestinal type and 14 of the cases (37% ) were diffuse type carcinomas. We diagnosed gastritis in all of the cases: 18 of them (47% ) were chronic atrophic gastritis, 8 of the cases (21% ) were Helicobacter pylori gastritis and 14 of the cases (37% ) were acut or chronic nonspesiphic gastritis. 19 of the cases (50% ) were intestinal metaplasia , 10 of them (26% ) were displasia and 8 of the cases (21% ) were regeneration. Conclusion: As a result, knowledge of these precursor lesions is fundamental to control and prevent gastric carcinomas.
