Spinal Anestezide Propofol ve Midazolamın Oksidatif Stres Parametreleri Üzerine Etkileri

Özet
Amaç: Spinal anestezi sırasında sedasyon amacıyla kullanılan birçok intravenöz anestezik ilacın serbest radikallerle etkileşimi bilinir. Bu çalışmada, spinal anestezide sedasyon sağlamak amacıyla kullanılan propofol ve midazolamın oksidatif stres üzerine etkileri postoperatif total antioksidan seviye (TAS), total oksidan seviye (TOS)ve oksidatif stress indeks (OSI)düzeyleri ile araştırıldı. Materyal ve metod: Çalışmaya 30-60 yaş arası, ASA risk 1 ve 2 grubu, elektif alt ekstremite operasyonu planlanan 40 hasta dahil edildi. Operasyon odasına alındıktan sonra, tüm hastaların kalp atım hızı, noninvaziv kan basıncı, periferik oksijen satürasyonu monitorizasyonu yapıldı. Bu monitorizasyon parametreleri 5 dk ara ile ölçülüp kaydedildi. Duyusal blok T10 seviyesine ulaştıktan sonra, Grup 1 (Grup P, n:20) hastalara propofol 1mg/kg bolus 10 dk'da, Grup 2 ( Grup M, n:20) hastalara da midazolam 0.05 mg/kg bolus uygulandı. İhtiyaç halinde aynı dozlar tekrar yapıldı. Tüm hastalardan postoperatif2. saatte 5 mlvenöz kan alındı. TAS, TOS ve OSI değerleri ölçüldü. Bulgular: Gruplar demografik özellikleri açısından benzerdi. Yaş, cinsiyet, ağırlık ve operasyon süreleri bakımından gruplar arasında istatistiksel olarak anlamlı fark yoktu (p>0.05). Gruplar arası postoperatif TAS, TOS ve OSI değerleri karşılaştırıldığında istatistiksel olarak anlamlı bir fark bulunmadı (p>0,05). Sonuç:Spinal anestezide sedasyon amacıyla kullanılan propofol ve midazolamın oksidatif stres sistem üzerine olumlu etkileri olduğu tespit edildi.Bu bulguların daha geniş kapsamlı ve uzun süreli çalışmalarla desteklenmesine ihtiyaç vardır.
Abstract
Background: Many intravenous anesthetics used for sedation during spinal anesthesia are known to interact with free radicals.In this study, the effects on the oksidative stress of propofol and midazolam used to provide sedation in spinal anesthesia was investigated with postoperative total antioxidan status (TAS), total oxidant status (TOS) and oxidative stress index (OSI) levels. Methods:Forty patients who elective lower extremity operations, between 30-60 years, ASA risk group 1 and 2 enrolled to the study.After being taken into the operating room, all of the patients were monitorized heart rate, noninvasive blood pressure, peripheral oxygen saturation. This monitoring parameters were measured and recorded every 5 minutes. After reaching the sensory block T10 level, Group 1 (Group P, n = 20) patients received propofol 1 mg / kg bolus 10 min, Group 2 (Group M, n = 20) patients received midazolam 0.05 mg / kg bolus. The same doses were repeated in case of need. From all patients were taken 5 mLvenous blood in postoperative 2 hours.TAS, TOS and OSI levels were measured. Results: The demographic characteristics of groups were similar.Age, gender, weight and operative times were not statistically significant difference among groups (p> 0.05). There were no significantly difference in postoperative TAS, TOS, OSI levels between groups. Conclısion: It was found positive that effects on antioxidant system of propofol and midazolam used for sedation during spinal anesthesia. These findings are needed to be supported by more comprehensive and long-lasting studies.
