Mastoidektomi sonrası gelişen dura beyin herniyasyonlarının cerrahi tedavisi: Olgu sunumu

Özet
Özellikle komplikasyonlu kronik otit cerrahisi geçiren hastalarda, timpanik kavitede görülen pulsatil kitle ve BOS sızıntısında, öncelikle dura-beyin herniyasyonu olasılığı akla gelmelidir. Bu olgular, en iyi şekilde, operasyon mikroskobu ve magnetik rezonans görüntüleme (MRG) ile değerlendirilebilirler. Sunulan olgularda kolesteatomlu kronik otit komplikasyonu 2 beyin absesi ve 1 sinüs tromboflebiti nedeniyle yapılmış radikal mastoidektomi kavitelerinde oluşan dura-beyin herniyasyonları, kemik otogreft ve temporal kas-periost flebi ile onarılmıştır.
Abstract
After surgery for complicated chronic otitis media, if a pulsatile mass and CSF leakage appear in tympanic cavity, dural and brain tissue herniation must be remembered first. These cases can be evaluated with operation microscope and magnetic resonance imaging (MRI) best. The study included two brain abscesses and one sinus thrombophlebitis cases that occurred as a complication of chronic otitis media that were treated by radical mastoidectomy. Dural and brain herniation in radical mastoidectomy cavities were treated surgically with bone autograft and temporal muscle-periosteal flap technique.
