Evaluation of The Patients With Sleeve Gastrectomy Due to Morbid Obesity

Özet

Objective : In our study, we investigated to what extent laparoscopic sleeve gastrectomy (LSG) operation, which is the most widely preferred among bariatric surgical procedures, changed the rates of obesity-related mortality, morbidity, drug use and complications. Materials and methods: In our clinic,431 patients who underwent LSG operation due to morbid obesity between April 2008 and June 2016 were retrospectively examined.We investigated in terms of their body mass index (BMI) values, the changes of excessive weight loss percentage, age, sex, hospital stay, additional diseases, changes in drug use due to additional diseases, complications of surgical procedure and mortality rates. SPSS statistical program and Paired sample t test were used in data analysis. Values with P <0.05 were considered statistically significant. Results: The average age of patients was 36.68 ± 10.4 years, of which 335 (77.7%) were female and 96 (22.3%) were male, the hospitalization average was 6.24 ± 4.98 days, preoperative period, postoperative 3rd month, 6th month, 12th month, 24th month and the lastaverage BMI values were observed according to follow-up time averages. Statistically a significant difference was revealed between preoperative and postoperative months (p <0.05). In postoperative period, significant reductions in comorbid diseases and drug use due to comorbid diseases were observed Conclusion: It has been observed that LSG, which is the most commonly used bariatric procedure, is effective in the treatment of obesity and its accompanying comorbid diseases, causing serious reductions in drug use rates due to these diseases. For this reason, we think that LSG significantly reduces the morbidity, mortality rates, financial and labor loss due to obesity worldwide.

Abstract

Amaç: Çalışmamızda bariatrik cerrahi prosedürler içerisinde en yaygın olarak tercih edilen laparoskopik sleeve gastrektomi (LSG) operasyonunun ,obeziteye bağlı oluşan mortalite,morbidite ,ilaç kullanım ve komplikasyon oranlarını ne oranda değiştirdiği araştırıldı. Gereç ve Yöntemler: Kliniğimizde Nisan 2008-Haziran 2016 tarihleri arasında morbid obezite nedeniyle LSG operasyonu yapılan 431 hastanın retrospektif olarak ;vücut kitle indeksi (VKİ) değerleri , aşırı kilo kaybı yüzdesi değişimleri,yaş,cinsiyet,hastanede kalış süreleri,ek hastalıkları,ek hastalıklara bağlı olarak ilaç kullanımındaki değişiklikler, yapılan cerrahi prosedürün komplikasyonları ve mortalite oranları araştırıldı. Verilerin analizinde SPSS istatistik programı ve Paired sample t test kullanıldı. P<0,05 olan değerler istatistiksel anlamlı olarak kabul edildi. Bulgular: 431 hastanın yaş ortalaması 36.68 ± 10.4 yıl olup bu hastaların 335'i ( %77.7) kadın ,96'sı (%22.3) erkekti, hastanede yatış gün ortalaması 6.24±4.98 gün olarak belirlendi, pre operatif dönem ile postoperatif 3.ay ,6.ay,12.ay, 24.ay ve takip süre ortalamalarına göre en son VKİ değer ortalamaları görüldü. Preoperatif dönem ile postoperatif karşılaştırılan aylar arasında istatistiksel olarak anlamlı fark saptandı.(p<0.05).Postoperatif dönemde komorbid hastalıklarda ve komorbid hastalıklara bağlı ilaç kullanımında ciddi azalmalar gözlendi. Sonuç: Günümüzde en sık uygulanan bariatrik prosedür olan LSG’nin obezite ve eşlik ettiği komorbid hastalıkların tedavisinde etkili olduğu.Bu hastalıklara bağlı ilaç kullanım oranlarında ciddi azalmalara yol açtığı görülmüştür.Bu nedenle LSG ‘nin dünya çapında obeziteye bağlı gelişen morbidite,mortalite oranlarını, maddi ve iş gücü kaybını ciddi oranda azalttığını düşünmekteyiz.
