Varis Dışı Üst Gastrointestinal Kanama ile Başvuran Hastalarda Skorlama Sistemlerinin Değerlendirilmesi

Özet

Amaç: Varis dışı üst gastrointestinal (GI) kanamalı hastaların müdahele ihtiyacı ve klinik sonuçlarının değerlendirmesinde, Glasgow-Blatchford skoru (GBS), preendoskopik Rockall skoru (RS) ve endoskopik Rockall skorunun faydasını karşılaştırmak. Gereç ve Yöntemler: Retrospektif tek merkezli çalışmamıza, Ocak 2018-Aralık 2019 tarihleri arasında varis dışı üst GI kanama nedeniyle hastanemiz acil servisine başvuran ve ilk 24 saat içerisinde endoskopisi yapılan hastalar dahil edildi. Tüm hastalar için GBS, preendoskopik RS ve endoskopik RS prospektif olarak hesaplandı. Hastaların müdahele ve kan transfüzyon ihtiyacını ve 30 günlük mortaliteyi belirlemede skorlama sistemlerinin doğruluğunu değerlendirmek için, alıcı işlem karakteristiği (ROC) analizinde eğri altında kalan alan (AUC) analizlerine dayanarak optimal kesim değerleri hesaplandı. Bulgular: Çalışmaya 74’ü (%67,9) erkek, 109 hasta dahil edildi. Hastaların median yaşı 62 idi (aralık: 47,5-79). Endoskopik müdahele ihtiyacı için GBS kesim değeri >14 [AUC 0,63; %95 güven aralığı (GA): 0,530-0,719] olarak bulundu. Kan transfüzyon ihtiyacının tahmini için, GBS > 12 (AUC 0,68; %95 GA: 0,592- 0,773), preendoskopik RS > 2 (AUC 0,64; %95 GA: 0,549-0,735), endoskopik RS > 4(AUC 0,70; %95 GA: 0,614-0,792) kesim değerleri elde edildi. GBS skoru ile hastanede yatış süresi arasında ilişki bulunmazken, yatış süresi ile preendoskopik RS (r=0,371, p=0,001) ve endoskopik RS (r=0,314, p=0,001) arasındaki ilişki anlamlıydı. 30 günlük mortaliteyi öngörmede endoskopik RS >7 en uygun kesim değeriydi (AUC 0,77; %95 GA: 0,688-0,852). Sonuç: Sonuçlarımız, endoskopik müdaheleyi tahmin etmede GBS’nin, 30 günlük mortaliteyi tahmin etmede ise endoskopik RS'nin etkinliğinin olduğunu göstermektedir. Kan transfüzyon ihtiyacını belirlemede GBS, preendoskopik RS ve endoskopik RS benzer ayırt edici kabiliyete sahiptir. Sonuçların klinik uygulamada kullanılabilirliğini doğrulamak için büyük prospektif çalışmalara ihtiyaç vardır.

Abstract

Objective & Aim: To compare the benefits of Glasgow-Blatchford score (GBS), preendoscopic Rockall score (RS), and endoscopic Rockall score in evaluating the need for intervention and clinical outcomes of patients with nonvariceal upper gastrointestinal (GI) bleeding. Material&Methods: Patients with nonvariceal upper gastrointestinal (GI) bleeding who were admitted to the emergency department of our hospital between January 2018 and December 2019 and have received endoscopy in the first 24 hours are included in this retrospective single-center study. For all patients, GBS, preendoscopic RS, and endoscopic RS were calculated prospectively. In order to determine the need for intervention, blood transfusion of the patients, 30-days mortality, and scoring systems’ accuracy receiver operating characteristic (ROC) analysis is used and optimal cut value is calculated. Results: 109 patients 74 of whom (67.9%) were male were included in the study. The median age of the patients was 62 (range 47.5-79). The GBS cut-off value for the need for endoscopic intervention was calculate to be > 14 (AUC 0.63; 95% CI: 0.530-0.719). For estimation of blood transfusion need, GBS > 12 (AUC 0.68; 95% CI: 0.592-0.773), preendoscopic RS> 2 (AUC 0.64; 95% CI: 0.549-0.735), endoscopic RS > 4 (AUC 0,70; 95% CI: 0.614-0.792) cut-off values were obtained. While there was no correlation between GBS score and length of hospital stay, the relationship between length of stay and preendoscopic RS (r = 0.371, p = 0.001) and endoscopic RS (r = 0.314, p = 0.001) was significant. Endoscopic RS > 7 was the most appropriate cut-off value for predicting 30-day mortality (AUC 0.77; 95% CI: 0.688-0.852). Conclusion: The results show that GBS is effective in estimating endoscopic intervention, and RS is effective in estimating 30-days mortality. GBS, preendoscopic RS, and endoscopic RS have similar discriminative ability to determine the need for blood transfusion. Large prospective studies are needed to verify the utility of the results in clinical practice.
