İdyopatik infertilitede klomifen sitrat tedavisinin yeri: İlk sonuçlar

Özet
Yardımcı üreme teknikleri idyopatik oligospermi ve /veya astenospermide en etkin tedavi seçeneği olarak kabul edilmektedir. Bu tekniklerin invazif oluşu, pahalılığı hastaların daha doğal ve ucuz olan tedavi seçeneklerine ilgisini artırmaktadır. Bu tedavilerden biri olan klomifen sitratın semen parametreleri üzerindeki etkilerini araştırmak amacıyla bu çalışma planlandı. İdyopatik infertilite tanısı alan 24 hasta çalışmaya dahil edildi. Hastalara 6 ay boyunca 50 mg/gün dozunda klomifen sitrat verildi. Tedavi öncesi ve sonrası sperm analizleri yapıldı ve tüm hastalar hormonal değerlendirmeye tabii tutuldu. Tedavi sonrası 13 hastada (%51.4) semen parametrelerinde iyileşme, 2 hastada (%15.3)sperm sayısında, 7 hastada (%53.8) motilitede, 4 hastada (%30.7) hem sayı hem motilitede düzelme saptandı. Klomifen sitrat ilk tedavi seçeneği olmamakla birlikte idyopatik infertil hastaların belli bir grubunda faydalı olmaktadır.
Abstract
Assisted reproductive techniques are the most effective choice of therapy in patients with idiopathic oligospermia and/or asthenospermia. Since these techniques are invasive and expensive, the tendency to the natural and cost effective therapy regimes offer various options. Clomiphene citrate is one of the recommended options. However, reports on its efficacy are controversial. 24 idiopathic infertile patients were included in this study. During the period of 6 months they took 50 mg/day clomiphene citrate. Semen parameters were obtained before and after therapy, also hormonal status were evaluated. Semen parameters of 13 patients(51.4%) improved after the therapy. Sperm count, motility and both parameters were improved in 2(15.3%), 7(53.8%) and 4(30.7%) patients, respectively. Though not a first choice of therapy clomiphene citrate is useful in a definitive group of idiopathic oligospermic patients.
