Romatoid artritli hastalarda romatoid faktör ve anti-ccp ilişkisi

Özet

Amaç: Romatoid artrit tanılı hastalarda romatoid faktör ile anti anti-CCP'in ilişkisinin araştırmak. Materyal Metod: İzmir Tepecik Eğitim ve Araştırma Hastanesi Fizik Tedavi Polikliniğinde Temmuz 2012-Temmuz 2013 tarihleri arasında izlenen RA tanılı hastalar çalışmaya alındı. Hastaların dosyalarından demografik özellikleri ve izlem süre- sindeki fizik muayene bulguları kaydedildi. Hastaların akut faz reaktanları olan eritrosit sedimantasyon hızı (ESH) ve C- reaktif protein(CRP), RF, anti-nükleer antikor (ANA) ve anti-CCP düzeyleri çalışıldı. Bulgular: Yaş ortalaması 49,4 ± 14,5 yıl olan 36 hastanın 2'si erkek (%5,6), 34'ü (% 94,4) kadındı. Hastaların 24(%66,7)'sinin el küçük eklemleri,7( %19,4)'sinin üst kök eklemleri tutulmuştu. ESH ve CRP ortalamaları sırasıyla 30,8 ± 18,9 ve 1 ± 0,7 olarak saptandı. Hastaların 26(%72,2)'sında RF,17( %47.2)'sinde ANA ve 31(%86,1)'inde anti-CCP pozitif bulundu. RF pozitif olan hastaların %81,5'inde anti-CCP pozitif, RF (-) olan hastaların %100 'unda anti-CCP pozitif saptan- mıştır. Sonuç: Romatoid artritin erken tanısında anti-CCP testinin değerlendirilmesi önemlidir. Eklem ağrısı ile gelen RF negatif hastalarda mutlak anti-CCP test bakısı gerekir.

Abstract

Aim: To investigate the relationship between rheumatoid factor (RF) and anti cyclic citrullinated peptide (anti-CCP) levels of the patients with rheumatoid arthritis (RA). Material and Method: The patients followed up at İzmir Tepecik Teaching Hospital, Clinic of Physical Therapy Department between July 2012-July 2013 by the diagnosis of RA were included. Demographic characteristics and physical examination find- ings obtained from the patient files were recorded. Acute-phase reactants (erythrocyte sedimentation rate and C-reactive protein), RF, anti-nuclear antibody (ANA) and anti-CCP levels were determined. Results: The mean age of the 36 patients was 49.4 ±14.5 years and 2 male and 34 (%94,4) were female. The involvement of small joints of the hands were 66.7% and upper stem joints were 19.4%. The mean sedimentation rate and C-reactive protein levels were 8.30±9.18/h and 1± 0.7, respectively. The frequency of RF positivity was 72.2%, ANA 47.2% and anti-CCP 86.1%. Anti-CCP was positive at 81.5% of RF positive patients; while positive for all RF negative cases. Conclusion: It is important to evaluate the anti-CCP test for early diagnosis of rheumatoid arthritis. Examination of anti-CClevels of RF (-) patients admitted by the symptom of joint pain should be required.
