Prognostic factors in stage III endometrial carcinoma: An analysis of 36 cases

Özet

Endometriyum kanseri gelişmiş ülkelerde en sık görülen jinekolojik malignitedir ve hastaların %15-25'i evre III endometrium kanseridir. Bu çalışmanın amacı cerrahi evre III endometrium kanserinde prognostik faktörleri belirlemektir. 36 Evre III endometrium kanseri olgusu retrospektif olarak incelendi. Hastalıksız sağkalım (DFS) ve total sağkalım Kaplan-Meier yöntemi kullanılarak hesaplandı. Prognostik faktörleri değerlendirmek için log rank testi ve Cox regresyon analizi kullanıldı. Yedi hasta evre III A ve 29 hasta evre III C idi. Bütün olgular dikkate alındığında DFS %60.2 ve beş yıllık sağkalım %65'di. 10 hastada nüks görüldü (III A 1, III C 9). Paraaortik nod metastazı olan hastalarda nüks riski 5.8 kez artmış olarak bulundu. Beş yıllık total sağkalım evre III A ve evre III C için sırası ile %83.3 ve %52.7 bulundu. Evre III endometrium kanseri vakalarında paraaortik lenf nodu metastazının en önemli risk faktörü olması lenf nodu disseksiyonunun önemini bir kere daha göstermektedir.

Abstract

OBJECTIVE: Endometrial cancer is the most common gynecological malignancy in developed countries and 15-25% of patients have stage III endometrial cancer. The purpose of this study was to determine the prognostic factors in surgical stage III endometrial carcinoma. STUDY DESIGN: 36 stage III endometrial cancer cases were analysed retrospectively. Disease free survival (DFS) and overall survival were calculated using the method of Kaplan-Meier. Log rank test and Cox regression analysis were used to evaluate prognostic factors. RESULTS: Seven patients were stage III A and 29 patients were stage III C respectively. DFS is 60.2% and five year overall survival is 65% when all of the cases taken into account. Recurrence is seen in 10 cases (III A 1, III C 9). The risk of recurrence is 5.8 times higher in patients with paraaortic node metastasis. CONCLUSION: Five year overall survival in stage III A and III C are found as 83.3% and 52.7% respectively. Determination of paraaortic lymph node metastases as the most important risk factor in stage III endometrial cancer cases shows the importance of lymph node dissection once again.
