Özet
Tanısı konmamış asemptomatik vertebral patolojiler, epidural ve spinal anestezi esnasında ve sonrasında semptomatik hale gelebilir ve gelişen komplikasyon anestezi ile ilişkilendirilebilir. Jinekolojik bir operasyon geçirecek 25 yaşındaki bayan hasta preanestezik vizit için başvurdu. Operasyon için genel anestezi istemeyen olguya epidural anestezi önerildi. Preanestezik vizitte, birkaç ay önce bir yüksekten düşme öyküsü dışında bir soruna rastlanmadı. Epidural anestezinin riskleri anlatılırken asemptomatik vertebral patolojilerden kaynaklanabilecek potansiyel riskler konusunda bilgilendirildiğinde, hastanın isteğiyle lumbosakral manyetik rezonans görüntüleme yapıldı. Görüntüleme ile L4_5 seviyesinde ileri derecede ekstrude disk hernisi tanısı konuldu. Beyin cerrahisinin önerisiyle acil diskektomi operasyonuna alındı. Bu olgu, nöroaksiyal anestezi için aydınlatılmış onam alınırken, tanısı konmamış asemptomatik vertebral patolojilerin de göz önünde bulundurulmasının medikolegal açıdan önemini vurgulamaktadır.
Abstract
Undiagnosed asymptomatic vertebral pathologies may be revealed during or following neuroaxial blocks, and anesthesiologist may be held responsible with this. A 25-year-old woman scheduled for an elective gynecological surgery applied for preanesthetic visit. There was no past neurological history except a fall several months before. While informing her about the risks of the epidural anesthesia, we mentioned the potential complication due to an undiagnosed asymptomatic vertebral abnormality. Therefore, she requested that a magnetic resonance imaging of her lumbosacral spine be done. The magnetic resonance imaging revealed a large extruded disc herniation at the level of L4-L5, An emergency herniectomy was recommended by the neurosurgery department, and a surgical decompression with laminectomy was performed. This case highlights the medico-legal importance of taking undiagnosed asymptomatic vertebral pathologies into consideration.
