Katarakt ameliyatı sonrası fibrin membran tedavisinde doku plazminojen aktivatörünün klinik kullanımı

Özet
Amaç: Komplikasyonsuz katarakt cerrahisi sonrası maksimum topikal ve sistemik antiinflamatuar tedaviye cevap vermeyen ciddi ön segment fibrin membran reaksiyonlarında ön kamaraya verilen doku plazminojen aktivatörünün (tPA) etkinliğini araştırmak. Gereç ve Yöntemler: Komplikasyonsuz katarakt cerrahisi sonrası fibrin membran reaksiyonu olan ve maksimum antiinflamatuar tedaviye cevap vermeyen 20 olguda ön kamaraya 25 μg/ml 0.1 cc tPA uygulandı. Uygulama 8-30. günler arasında yapıldı. tPA enjeksiyonu sonrası topikal kortikosteroid ve sikloplejik tedaviye devam edildi. Olgular enjeksiyon sonrası 1., 2. ,3. saatte, 1., 2., 3. günde ve 1. ,2. ,3. ,4. haftalarda fibrin membranda çözülme, görme keskinliği ve göz içi basıncı yönünden değerlendirildi. Bulgular: Ön kamaraya tPA uygulanması sonucunda 20 olgunun 12'sinde (%60) fibrin membranda tam düzelme, 6'sında (%30) kısmi düzelme oldu. Kısmi düzelme olan olgularda 1 hafta ek antiinflamatuar tedaviyle tam düzelme gerçekleşti. Enjeksiyon sonrası kısmi düzelme olan 1 olguda (%5) 2. tPA uygulanması sonrası tam düzelme olurken, 1 olguda (%5) herhangi bir düzelme saptanmadı. Pupiller blok ile açı kapanması gelişmiş olan 3 olguda tPA sonrası oluşan fibrinolizisle birlikte göz içi basınçları enjeksiyonun ilk gününde normal düzeye indi. tPA'nın etkin olduğu 12 olgu (%60) ve tekrar tPA uygulanan 1 olguda (%5) görme düzeylerinde artış oldu. tPA'nın etkin olmadığı 1 olguda tPA sonrası medikal antiglokomatöz tedaviye cevap veren geçici göz içi basınç artışı oldu. Sonuç: Ön kamaraya düşük doz tPA verilmesi katarakt cerrahisi sonrası oluşan persistan fibrin membran tedavisinde etkin ve güvenilir bir yöntemdir.
Abstract
Purpose: To evaluate the efficiency of the use of intracameral tissue plasminogen activator (tPA) for severe anterior segment fibrin membrane formation following uneventful cataract surgery that fails to respond to maximum topical and systemic anti-inflammatory therapy. Material and Methods: 0.1 cc of 25 μg/ml tPA was applied to the anterior chamber in 20 patients who had fibrin membrane formation following uneventful cataract surgery that failed to respond to maximal anti-inflammatory treatment. The intervention is done in the 8th to 30th postoperative days. The dissolution of fibrin membrane, visual acuity and intraocular pressures were assessed on 1., 2., 3. hours, 1., 2., 3., days and 1., 2., 3. weeks following the tPA injection. Results: After intracameral tPA application, 12 cases out of 20 (60%) showed complete dissolution of the fibrin membrane. 6 cases (30%) had a partial improvement and these cases had complete resolution with additional anti-inflammatory therapy. 1 case (5%) showed no improvement while another case (5%) had complete resolution after a second tPA application. In 3 cases that had angle closure with pupillary block, intraocular pressure dropped to normal levels one day after tPA. Visual acuity improved in 12 cases (60%) on whom tPA was effective and in 1 additional case (5%) that had tPA re-application. In one case that was resistant to tPA, intraocular pressure following tPA had a transient increase that had responded to anti-glaucomatous therapy. Conclusion: Low dose intracameral tPA is an effective and safe method for persistent fibrin membrane formation following cataract surgery.
