Özet
Amaç: Rektal prolapsus tedavisinde çok sayıda cerrahi yöntem tanımlanmıştır. Abdominal rektopeksi ameliyatlarının anatomik başarısı iyi olmasına karşılık, postoperatif dönemde ciddi kabızlığa neden olabilmektedir. Kliniğimize aynı dönem içinde başvuran, biri daha önceden prolapsus nedeniyle ameliyat edilmiş, üç rektal prolapsus hastasında konu teknik olarak irdelendi. Olgu sunumları: İlk olguya düzenli defekasyon alışkanlığından dolayı sadece posterior rektopeksi, ikinci olguya preoperatif kabızlık yakınmasından dolayı posterior rektopeksi ile birlikte sigmoid kolon rezeksiyonu uygulandı. Üçüncü olguya ise 10 yıl önce geçirilmiş posterior rektopeksi ameliyatı sonrasında gelişen kronik kabızlık nedeniyle sigmoid kolon rezeksiyonu eklendi. Hastaların ameliyat sonrası erken dönem izlemlerinde kabızlık sorunu, nüks veya başka komplikasyon gözlenmedi. Sonuç: Erken dönem sonuçlarının uzun vadede de sürmesi halinde, burada sunulan üç farklı vakaya yaklaşımın rektal prolapsus cerrahisi sonrası görülebilecek olası problemlerin önlenmesi açısından iyi birer örnek olduğu düşüncesindeyiz.
Abstract
Objective: Several surgical methods have been described for the treatment of rectal prolapse. In spite of their anatomical success, abdominal rectopexy procedures have been associated with significant postoperative constipation. Within the same period, three patients with rectal prolapse (one of them treated previously) necessitated different surgical strategies. Case reports: The first patient underwent posterior rectopexy only, which was indicated due to the regular bowel habits. Because of the co-existing constipation, the second patient underwent a posterior rectopexy with sigmoid colon resection. The third patient was performed sigmoid colon resection also, who has constipation since posterior rectopexy operation performed 10 years ago. Neither of the patients had constipation, recurrence or another complication during the early postoperative period. Conclusion: If the early results of the patients operated with different surgical procedures for rectal prolapse go on well in the long term, the discussed surgical techniques can be good choices in preventing the problems that can be seen during rectal prolapse surgery.
