Akut hepatit A vakaları ve ekstrahepatik komplikasyonlar

Özet
Amaç: Akut hepatit A tanısı alan 168 vakanın klinik özelliklerinin ve ekstrahepatik komplikasyonlarının belirlenmesi. Yöntem: 1999-2003 yılları arasında akut hepatit A tanısı ile yatırılarak izlenen 168 vakanın kayıtları retrospektif olarak incelendi. Bulgular: Vakaların 70'i kız, 98'i erkekti (E/K=1.4). Yaş ortalaması 8.4±3.1 (2-17) olarak bulundu. Hastaların % 17.8'inde hepatit A infeksiyonu geçiren vakalar ile yakın temas öyküsü vardı. Vakaların 58'inde (% 34.5) ekstrahepatik komplikasyonlar, % 9.5'inde anemi, % 11.3'ünde trombositopeni, % 9.5'inde lökopeni, % 1.7'sinde ürtiker, % 1.7'sinde asit, % 0.6'sında akut glomerulonefrit, % 0.6'sında plevral efüzyon + asit gelişti. İki vakada akut hepatit A'nın atipik formu olan kolestatik hepatit, bir vakada fulminan hepatit gelişti. Sonuç: Çalışmamız, akut hepatit A infeksiyonunun halen ülkemizde halen önemli bir halk sağlığı sorunu olduğunu göstermektedir. Çocuklarda infeksiyonun seyri sırasında farklı organ ve sistemlerde birçok komplikasyon gelişebilir.
Abstract
Aim: To determine the clinical features and extrahepatic complications of patients with acute hepatitis A infection Methods: Files of 168 patients with acute hepatitis A infection diagnosed between 1999 and 2003 were evaluated retrospectively. Results: The mean age of children (mean±SD) was 8.4±3.1 years (age range: 2 to 17 years). Seventy of them were girls and ninety-eight were boys. Extrahepatic complications of the disease were anemia in sixteen (9,5 %), thrombocytopenia in nineteen (11,3 %), leukopenia in sixteen (9,5 %), urticaria in three (1,7 %), ascites in three (1,7 %), acute glomerulonephritis in one (0,6 %), pleural effusion and ascites in one of the cases (0,6 %). A typical forms were cholestatic hepatitis A in two (1,2 %) and fulminant hepatitis in one of the cases (0,6 %). Conclusion: Our study showed that childhood acute hepatitis A infections is still an important public health problem in our country. Many complications of different organ systems may be observed during the course of this infection.
