Diyabetik hastalarda katarakt cerrahisinde tek doz subtenon triamsinolon asetonid enjeksiyonu’nun kistoid maküler ödem gelişimi üzerine etkisi

Özet

Amaç: Bu çalışmada diyabetik hastalarda tek doz subtenon triamsinolon asetonid (TA) enjeksiyonunun katarakt cerrahisi sonrası kistoid maküler ödem (KMÖ) gelişimi üzerine etkisini optik koherens tomografi (OKT) ile değerlendirmeyi amaçladık. Gereç ve Yöntem: Bu geriye dönük, randomize, kontrollü çalışmaya fakoemülsifikasyon tekniği ile komplikasyonsuz ka- tarakt cerrahisi ve kapsül içi göz içi lens implantasyonu yapılan ve tip 2 diyabet tanısı bulunan 31 hastanın 40 gözü alın- mıştır. Subtenon grubunda hastalara tek doz posterior subtenon triamsinolon asetonid enjeksiyonu yapılırken, kontrol grubunda triamsinolon asetonid enjeksiyonu yapılmadı. En iyi düzeltilmiş görme keskinliği (EDGK), göz içi basıncı (GİB), OKT ölçümlerinde KMÖ gelişimi iki grup arasında ameliyat öncesi ve sonrası 1. hafta, 1. ay karşılaştırıldı. Bulgular: Subtenon grubunda 20 göz, kontrol grubunda 20 göz çalışmaya alındı. Takiplerde iki grup arasında EDGK bakımından istatistiksel anlamlı bir fark saptanmadı (p>0.05). Kontrol grubunda 1. ayda 4 gözde KMÖ görülürken, subte- non grubunda hiçbir olguda KMÖ saptanmadı (p=0.048). Her iki grupta topikal tedavi gerektiren intraoküler basınç artışı izlenmedi. Sonuç: Diyabetik hastalarda tek doz subtenon triamsinolon asetonid enjeksiyonu ile fakoemülsifikasyon cerrahisi sonrası KMÖ gelişiminde azalma olmakla birlikte subtenon enjeksiyonun görme keskinliği ve intraoküler basınca etkisi gözlenmemiştir.

Abstract

Purpose: In this study, we aimed to assess the effectiveness of a single sub-tenon injection of triamcinolone acetonide on the development of cystoid macular edema (CME) detected by ocular computed tomography (OCT) in diabetic patients after cataract surgery. Materials and Methods: In this prospective, randomized controlled study, we included 40 eyes of 31 patients with type 2 diabetes mellitus who underwent phacoemulsification and intracapsular lens implantation without any complication. The con- trol group comprised eyes not treated with a triamcinolone acetonide injection, and the subtenon group comprised eyes treated with a single posterior sub-tenon injection of triamcinolone acetonide. Best corrected visual acuity, intraocular pressure (IOP), CME development were compared between the two groups preoperatively and at postoperative 1st week, 1st month. Results: The subtenon group comprised 20 eyes and the control group, 20 eyes. There was no statistically significant dif- ference between the 2 groups in the mean corrected distance visual acuity at any follow-up examination (p>0.05). At post- operatif 1st month, four eyes in the control group and no eye in the subtenon group developed CME (p=0.048). IOP was not developed in any eyes which necessitated topical medication. Conclusion: Although, a single posterior subtenon injection of triamcinolone acetonide significantly reduced the incidence of CME in diabetic patients after phacoemulsification surgery, no effect on visual acuity and IOP was observed.
