Özet

Araştırmamızda, pulmoner tüberküloz (PT) ve ekstrapulmoner tüberküloz (EPT) şüpheli klinik örneklerde, mikobakterilerin varlığını kısa sürede saptamaya yönelik olarak kullanılan \"Mycobacteria Growth Indicator Tube (MGIT)\" sisteminin, referans yöntem \"Löwenstein-Jensen (LJ)\" ile karşılaştırılması ve izole edilen suşların 4 majör anti-mikobakteriyel ilaca karşı dirençlerinin belirlenerek, olguların primer ve sekonder dirençlerinin değerlendirilmesi amaçlanmıştır. Değişik kliniklerden klinik, radyolojik, histopatolojik ve immünolojik bulgulara göre PT veya EPT şüpheli 648 örnek incelemeye alındı. Örneklerden öncelikle \"Ziehl-Neelsen (ZN)\" boyama, daha sonra da standart bakteriyolojik kurallara göre LJ besiyerine ve üretici firmanın öngördüğü şekilde MGIT sistemine kültür yapıldı, izole edilen mikobakteriyel suşların tip tayininde geleneksel biyokimyasal testler ve BACTEC sisteminde NAP (p-nitro-alpha-acethylamino-beta-hydroxypropiophene) kullanıldı. BBL-MGIT antibiyotik duyarlılık testi (ADT) ile streptomisin (S), izoniazid (H), rifampin (R), etambutole (E) (SIRE) karşı duyarlılık testi yapılarak, herhangi bir ilaca karşı dirençli bulunan suşların ayrıca ağar orantılama yöntemiyle dirençleri konfirme edildi. Altıyüzkırksekiz örneğin 61 (%9.4)\'inde Mycobacterium spp. üretilmiştir. Tipleme tayini sonunda 61 susun 58\'i Mycobacterium tuberculosis, diğer 3 sus M. tuberculosis kompleks (MTK) olarak bulunmuştur. Kültürde üreme olan örneklerin 32\'sinde ZN pozitif olup bu olgularda MGIT üreme zamanı 12.2, LJ\'de 24.1 gün olarak tespit edilmiştir (p< 0.001). ZN negatif 29 örnekte ise MGIT üreme zamanı 23, LJ\'de ise 37 gündür (p< 0.001). Otuzdokuzu yeni, 22\'si eski hasta olan 61 olgunun 23 (%37.7)\'ünde ilaç direnci görülmüş, direncin 8 (%20.SI)\'i primer, 15 (%68.I8)\'İ sekonder olarak tespit edilmiştir. Tek ilaca karşı hem primer hem de sekonder direnç belirlenirken, 2 ilaç direncinde sadece H + R kalıbında (4 olguda) sekonder direnç belirlenmiştir. Kümülatif en yüksek ilaç direnci hem primer hem sekonder olarak H\'ye karşı belirlenmiştir. Sonuç olarak araştırmamızda, MGIT\'nin yüksek duyarlılık ve özgüllükte olması, LJ\'ye göre de kısa sürede sonuç vermesi, antibiyotik duyarlılık testinin yapılabilmesi ve uygulamasının kolay olması, tüberkülozun hızlı bakteriyolojik tanısı ve antimikrobiyal direncinin belirlenmesine yönelik bir avantaj olarak değerlendirilmiş ve rutin laboratuvar çalışmalarına girmesinin önemi vurgulanmıştır. Aynı zamanda araştırmamızda bulunan sekonder dirençle ilgili yüksek oran toplumda tüberküloz tedavisindeki uyumsuzluk ve yetersizliği düşündürmekte, bunun da yeni kontaminasyonlarla primer tüberküloz oranını ciddi olarak arttırabileceğini akla getirmektedir.

Abstract

In our study, it is aimed to compare the M/cobacteria Growth Indicator Tube (MGIT) which detects the presence of Mycobacteria rapidly with the reference method Löwenstein-Jensen Lj in clinical samples having suspect of pulmonary and extrapulmonary tuberculosis and to determine the resistance of the isolated strains against four major antimicrobial drugs and to evaluate the primary and secondary resistances of the cases. Sixhundred and fortyeight clinical samples from different clinics suspected of pulmonary tuberculosis (PT) or extrapulmonary tuberculosis (EPT) according to the clinical, radiological, histopathological and immunological findings were investigated. The samples were first stained with Ziehl-Neelsen (ZN) method, then, cultured in LJ according to the standard bacteriological rules and in MGIT tubes as recommended by the manufacturer. Conventional biochemical tests and NAP (p-nitro-alpha-acethylamino-beta-hydroxypropiophene) of BACTEC System were used to identify isolated mycobacterial strains. The BBL-MGIT antimicrobial susceptibility test (AST) was performed to detect susceptibility to streptomycine (S), isoniazid (H), rifampicin (R), ethambutole (E) (SIRE) and the strains which were found resistant to any of the drugs were confirmed by performing agar proportion method. Mycobacterium spp. were isolated in 61 (9.4%) out of 648 samples. After species identification, 58 out of 61 strains were found as M. tuberculosis and other three as M, tuberculosis complex (MTC). Thirty-two samples which were isolated in the culture were ZN positive and the growth time was 12.2 days in MGIT, 24.1 days in LJ (p< 0.001). Twenty-nine were ZN negative and their growth time was 23 days in MGIT, 37 days in LJ (p< 0.001). In 61 cases consisting of 39 newly and 22 pleviously diagnosed patients, drug resistance were seen in 23 (37.7) which were 8 (20.51 %) as primary and 15 (68.18%) secondary resistance. In one drug resistance, both primary and secondary resistance were detected whereas in 2 drug resistance, secondary resistance was seen in only H + R pattern (4 cases). The highest cumulative drug resistance was found against H as both primary and secondary resistance. Our study emphasizes that MGIT has high sensitivity and specificity, performs antimicrobial susceptibility test, is easy to perform and is a more rapid method than LJ, so it is evaluated as an advantage in rapid bacterial identification of tuberculosis and in establishing antimicrobial\' resistance the importance of its application in routine laboratory work is underlined. In addition to these, our study makes us think that the high ratio of secondary resistance in public may be related to in compliance of patients or insufficiency of therapy and this may cause a serious raise of primary tuberculosis with new contaminations.
