Özet

Amaç: Skolyozla birlikte spinal kord patolojileri birlikte görülebilir. Omurga deformitesi düzeltilirken spinal kordda gerilmeye bağlı nörolojik defisit gelişmemesi için öncelikle kranioservikal veya lomber bölgeye yönelik, öncül cerrahi işlemlere gerek duyulabilir. Çalışmamızda kanal içi patolojilere yönelik daha önce operasyon uygulanmış ileri derecede omurga deformiteli hastalarda skolyoz cerrahisi sonrası sonuçlarımız değerlendirilmesi amaçlanmıştır. Hastalar ve metot: Bu çalışmaya 2006-2010 yılları arasında başvuran konjenital ve nöromusküler skolyozlu 141 (66 hasta konjenital, 75 hasta nöromusküler) hasta dâhil edilmiştir. Bu 141 hastanın manyetik rezonans (MR) incelemesinde; 52 hastada (% 36.8) 88 intraspinal anomali saptanmıştır. İntraspinal anomali olduğu belirlenen 52 hastanın 7 (% 4.9)ʼsinin kanal içi patolojileri nedeniyle başka merkezlerde nöroşirürji kliniklerince opere edildiği belirlenmiştir. Geri kalan 47 hasta nöroşirurjik değerlendirme ve tedavileri için başka merkezlere yönlendirilmiştir. Bu 7 hastadan 2ʼsi skolyotik eğrilikleri için operasyonu kabul etmemiş ve 5 (% 3.6)ʼya skolyotik deformiteleri için cerrahi tedavi uygulanmıştır. Skolyoz cerrahisi uygulanan hastaların ortalama yaşı 15.6 yıl (12.8-19), son kontrol tarihine kadar takip süresi ortalama 34 ay (21-50)ʼdır. Dört hastaya posterior düzeltme ve füzyon, 1 hastaya büyümeye izin veren rodlar ile non-füzyon tespit ve uzatma yapılmıştır. Bu çalışmada öncelikle çalışmaya dahil edilen 141 hastanın demografik ve etiyopatolojik verileri gözden geçirilmiştir. Ayrıca opere edilen hastaların son kontrollerinde koronal planda elde edilen düzelme miktarları, görsel ağrı cetveli (GAC) ile hasta memnuniyet skorları, nörolojik durumları, enfeksiyon ve implant yetmezliği olup olmadığı araştırılmıştır. Sonuçlar: Konjenital ve nöromusküler skolyozu olan 141 hastada%36.8 gibi yüksek bir oranda intraspinal anomali eşlik ettiği belirlenmiştir. Opere edilen hastalarda koronal planda preoperatif Cobb açısı ortalama 80.4º (33-101), son kontrolde ortalama 40.2º (13-72) indirildiği saptanmıştır. Düzelme oranı % 50.3 (28.8-70.1) olarak tespit edilmiştir. Postoperatif 1 olguda erken enfeksiyon görülmüş, yara debridmanı ile tamamen eradike edilmiştir. Nörolojik bozulma ve implant yetmezliği bulgusu ile karşılaşılmamıştır. Son kontrollerinde GAC skorlarının ortalama 10 olduğu belirlenmiştir. Sonuç: Bu çalışmada, literatür verileri ile uyumlu olarak konjenital ve nöromusküler skolyozlu hastalarda intraspinal anomali oranı oldukça yüksek (% 36.8) bulunmuştur. Bu nedenle konjenital ve nöromusküler skolyozu olan hastalarda mutlaka tüm vertebra MRʼlarının çekilmesi gerektiği fikri elde edilmiştir. Ayrıca bu hastalarda nöroşirurjik yaklaşım sonrası yapılacak cerrahi korreksiyon ile oldukça tatmin edici sonuçlar alınabileceği belirlenmiştir.

Abstract

Objective: Scoliosis may be observed with the pathologies of spinal cord. Prior to the correction of spinal deformities, first line surgical procedures may be necessary to prevent the neurological deficits that develop due to spinal cord contractions. Our study aims to evaluate the results of scoliosis surgeries in patients with advanced spinal deformities who had previously undergone surgical treatments for intraspinal pathologies. Patients and Method: The study included 141 patients with congenital (66 patients) and neuromuscular (75 patients) scoliosis, who applied during the years between 2006 and 2010. The magnetic resonance image (MRI) evaluations revealed 88 intraspinal anomalies in 52 (36.8%) of 141 patients. Of the 52 patients with intraspinal anomalies, 7 (4.9%) were operated in the other neurosurgical clinics due to intraspinal pathologies. The remaining 47 patients were referred to the other centers for neurosurgical evaluation and treatment. While two of the 7 patients did not accept the surgical procedure for scoliotic curves, 5 (3.6%) patients undergone surgery for scoliotic deformities. The mean age of the patients undergoing scoliosis surgery was 15.6 years (12.8 to 10 years), and the mean follow-up period until the last control was 34 months (21 to 50 months). Four patients underwent posterior correction and fusion, and one patient underwent non-fusion fixation and lengthening. This study reviewed the demographic characteristics and etiopathological data of 141 patients included in the study. Furthermore, the operated patients were evaluated for the degree of correction, patients' satisfaction by visual analog scale (VAS), neurological status and the presence of infection as well as implant failure, in their last controls. Results: Intraspinal anomalies accompanied in a high rate of 36.8% in 141 patients with congenital and neuromsucular scoliozis. The mean coronal preoperative Cobb angle was 80.4º (33º to 101º), and decreased to 40.2º (13º to 72º) in the last controls. Correction rate was 50.3% (28.8% to 70.1%). One patient experienced postoperative infection, which was completely cured by wound debridement. There were no findings of neurological degeneration nor implant failure. GAC scores were detected as 10 in the last controls. Conclusion: In line with the literature reports, this study revealed a quite high rate of intraspinal anomalies in patients with congenital and neuromuscular scoliosis. This raised the idea that spinal MRI evaluations are necessary for all patients with congenital and neuromuscular scoliosis. In addition, we concluded that quite satisfactory results can be obtained by surgical correction, following neurosurgical evaluations.
