Sevofluran ve desfluranın titrasyonunda bispektral indeks (BİS) kullanılmasının postoperatif derlenme üzerine etkileri

Özet
Amaç: Bispektral indeks (BİS), anestezi derinliğini ölçmek amacıyla geliştirilmiş ileri bir EEG parametresidir. Çalışmamızda, BİS monitörizasyonunun anestezi derinliğine ve derlenme kriterleri üzerine etkileri araştırıldı. Yöntem: Kolesistektomi operasyonu geçirecek 40 hasta randomize olarak 4 gruba ayrıldı. Standart indüksiyonu takiben, anestezi idamesi Grup V de sevofluran ve Grup 3'te desfluran titrasyonu ile BIS değerleri 40 ile 60 arasında olacak şekilde, Grup 2'de sevofluran ve Grup 4'te desfluran (kontrol grupları) titrasyonu, standart klinik pratiklere göre (hemodinamik veriler, pupil büyüklüğü ve gözyaşı varlığı gibi) sürdürüldü. Tüm hastalara operasyon boyunca remifentanil infüzyonu (0.125-0.25 mg kg'dk') verildi. Tüketilen remifentanil ve sisatrakuryum miktarları, derlenme süreleri ve Aldrete skorları kaydedildi. Bulgular: Gruplar arasında, hemodinamik veriler açısından fark saptanmadı. BİS gruplarıyla kontrol grupları arasında end-tidal gaz (ET-GAZ) değerleri yönünden fark yoktu. BİS ve minimum alveoler konsantrasyon (MAC) değerleri arasında sadece birkaç noktada görülen fark hemodinamik verilerde bir farklılığa yol açmadı. Tüketilen remifentanil ve sisatrakuryum miktarları, derlenme süreleri ve Aldrete skorları açısından fark saptanmadı. Sonuç: Anestezi uygulamasında derlenme süreleri hızlı olan sevofluran ve desfluran gibi inhalasyon ajanlarının, etkinliği yüksek ve yarılanma ömrü kısa bir opioid olan remifentanille birlikte tecrübeli anestezistlerce kullanılması durumunda, BİS monitörizasyonunun çok gerekli olmadığını düşünmekteyiz.
Abstract
Objective: Bispectral index (BIS) is an advanced electroencephalogram derived parameter to measure the depth of anaesthesia. We aimed to assess the effects of BIS monitoring on recovery and anesthetic depth. Method: Forty patients who were scheduled for colesistectomy were randomly divided into four groups. After induction, in Group 1 sevoflurane and in Group 3 desflurane were titrated to maintain a BIS value between 40 and 60 during surgery. In Group 2 sevoflurane and in Group 4 desflurane (control groups) were administered according to the standard clinical practice. All of the patients were infused remifentanil (0.125-0.25 mg kg'dk'1) intravenously during surgery. Result: There were no differences in the hemodynamic parameters of the groups. There were no differences between the BIS groups and the control groups in end-tidal gas (ET-GAS) values. In some periods, BIS and minimum alveolar concentration (MAC) values were different among the groups, although this difference did not cause any hemodynamic difference. There were no significant differences in the consumption of remifentanil and cisatracurium amounts, recovery times and Aldrete scores between the groups. Conclusion: We concluded that BIS monitoring is not essential if inhalational agents with early recovery profiles, such as sevoflurane and desflurane, and an effective and short- acting opioid, such as remifentanil, are used in anesthesia management by an experienced anesthesiologist.
