Özet

Amaç: Bu çalışmada lokal invaziv mesane kanserlerinde kemoradyoterapi (KRT) uygulaması ile sadece radyoterapi (RT) ve sadece kemoterapi (KT) uygulamalarının, akut ve geç toksisite, lokal kontrol ve sağ kalım üzerine etkilerinin retrospektif olarak karşılaştırılması amaçlandı. Yöntem: Şubat 1990-Ekim 1995 tarihleri arasında Şişli Etfal Hastanesi Radyasyon Onkolojisi Kliniğine başvuran 56 invaziv mesane kanserli olgu retrospektif olarak analiz edildi.AJCC’97 evrelemesine göre 12 olgu T2N0M0 (% 21), 22 olgu T3aN0M0 (% 39), 9 olgu T3N0M0 (% 16), 3 olgu T3a,bN1M0 (% 5), 5 olgu T4a,bN0,1M0 (% 9) e 5 olgu T3,4N1M1 (% 9) olarak değerlendirildi. Bu olgularda yaş, cinsiyet, histopatoloji, klinik evre parametrelerine göre KT, KRT ve RT gruplarının toksisite, lokal kontrol ve sağ kalım oranları karşılaştırıldı. Bulgular: Medyan yaş 62 (31-90 yaş) olup 50 yaş üzerindeki olgular tüm olguların % 87.5’i idi. Olguların % 96’sı TCC, % 4’ü skuamöz hücreli karsinomdu. En sık (% 95) başlangıç semptomu hematüridir. KT, KRT ve RT gruplarının lokal kontrol oranları sırasıyla % 40,% 88.2 ve % 82.6 olup KRT ve RT gruplarının arasında anlamlı bir fark bulunmadı. Sonuç: Lokal invaziv mesane kanserlerinde kemoradyoterpi (KRT) ve radyoterapi (RT) uygulamasının toksisite, lokal kontrol ve sağ kalım açısından “yeterli”, maliyet açısından da “uygun” bir tedavi olduğu kanaatine varılmıştır.

Abstract

Objective: In this study in local invasive bladder cancer, neoadjuvant chemoradiotherapy (KRT) application with the only radiotherapy (RT), and only chemotherapy (CT) applications, acute and late toxicity, local control and survival effects were aimed to compare. Methods: Between February 1990-October 1995 in 56 patients with invasive bladder cancer who applied to Sisli Etfal Hospital Radiation Oncology Clinic. A retrospective analysis of 12 cases compared AJCC '97 staging T2N0M0 (21%) and 22 patients T3aN0M0 (39%), 9 patients with T3N0M0 (16%), 3 cases, T3a,bN1M0 (5%), 5 cases T4A,bN0,1M0 (9%) and 5 cases, T3,4N1M1 (9%) were considered. In these patients age, gender, histopathology, clinical stage according to the parameter KT, KRT and RT groups, toxicity, local control and survival rates were compared. Results: Median age 62 (31-90) and 50 years on 87.5% of all cases of patients' in constitute. TCC 96% of cases, 4% of squamous cell carsinomas. The most common initial symptom was hematuria (95%). KT, KRT and RT groups, respectively, 40% local control rate, 88.2% and 82.6% and between KRT and RT groups showed no significant difference. Conclusion: Chemoradiotherapy and radiotherapy in invasive bladder cancer, the local toxicity, local control and survival in terms of “adequate”, the “appropriate” in terms of cost as a treatment has been concluded.
