Özet

Herpes simplex virüs keratiti, korneal enfeksiyon nedenli görme kayıplarına sebep olan bir hastalıktır. Virüs duyusal ganliyonlarda yerleşen ve tekrarlayan bir enfeksiyon oluşturur. Hastalığın tedavisinde oral ve topikal yolla uygulanan bir antiviral ajan olan Asiklovir kullanımı olumlu sonuçlar vermektedir. Bazı hastalarda hastalığın ataklarla seyrediyor olması, hekimleri hastalığın aktif olduğu dönemlerin dışında profilaktik amaçlı tedavi uygulamalarına yönlendirmiştir. Bu doğrultuda proflaktik olarak verilen düşük doz oral Asiklovir tedavisi yüz güldürücü sonuçlar vermektedir. Bu çalışmada, tekrarlayan herpetik keratit ataklarıyla karakterize olan 30 yaşında bir erkek hasta sunulmuştur. Hastanın atak sıklığı ve korneal dentritik yayılım paterni nedeniyle, proflaktik Asiklovir tedavisi günlük 800 mg belirlenmiştir. Dokuz yıllık takip süresi boyunca, proflaktik asiklovir tedavisi ile hastanın keratit ataklarında belirgin azalma gözlemlenmiş, ve olası görme kaybı önlenebilmiştir.

Abstract

Herpes simplex virus keratitis is a disease which is related to vision loss secondary to infection. Virus infection is settled on sensory ganglions and causes recurrent infections. Oral and topical administration of acyclovir as an antiviral agent may give positive results. Periods of active relapsing of the disease have directed the physicians outside the prophylactic treat- ment applications. Therefore, low-dose oral acyclovir maintenance therapy provides positive outcomes results. In this study, a thirty years old male patient who is characterized with recurrent herpetic keratitis is reported. The patient’s pattern of attack frequency and corneal dendritic propagation taken into consideration of a maintance dose of 800 mg daily oral acyclo- vir. In a follow-up period of 9 years, we have observed a significant reduction in the exabercations of keratitis and potential visual loss was avoided by the use of prophylactic acyclovir therapy.
