Subklaviyan ven kateterizasyonuna bağlı endotrakeal hemoraji ve hemotoraks

Özet
Santral ven kateterizasyonu, anatomik işaret noktalarının yardımı ile uygulanmaktadır. Son yıllarda ise ultrasonografi ile görüntüleme tekniğinin kullanılması önerilmektedir. Mitral kapak replasmanı için genel anestezi uygulanan olguda, işaret noktaları tekniği ile başarısız sağ internal juguler ven (İJV) kateterizasyon girişiminden sonra, sağ subklaviyan ven (SKV) kateterize edilmiş ve kateterizasyondan hemen sonra, endotrakeal tüpten aktif kanama gelişince acil sternotomi uygulanmıştır. İncelemede geçirilmiş enfeksiyonlara bağlı olarak, sağ akciğer üst lobunun plevra ve göğüs duvarına yapışık olduğu görülmüştür. Apekste parankim hasarı, subklaviyan ven etrafında hematom ve hemotoraks saptanmıştır. Parankim hasarı sütüre edilerek göğüs tüpü yerleştirilmiş ve açık kalp ameliyatı ertelenmiştir. Geçirilmiş enfeksiyona bağlı oluşan yapışıklıklar subklaviyan ven kanülasy onunda teknik zorluğa neden olmuş ve sonuçta kateter kılavuz telinin damar duvarını delip lümen dışına çıkmasına ve hemotoraksa neden olduğu düşünülmüştür. Bu olgudaki gibi zor santral ven kateterizasy onlarında, ultrasonografi ile görüntüleme tekniği uygulamasının başarıyı arttırıp komplikasyonları azaltacağı görüşündeyiz.
Abstract
Central vein catheterization is performed using anatomic landmark technique. Recently, ultrasound imaging technique is recommended when inserting central venous catheter. In this case, after failure of right juguler vein catheterization attempt with anatomic landmark technique, right subclavian vein (SCV) was catheterized under general anesthesia for mitral valve replacement (MVR). Active hemorrhage was observed through the endotracheal tube and emergent sternotomy was performed. Stiff adhesion of the right upper lung lobe to the pleura and thorax was observed. There were paranchimal lesions at the lung apex, haemothorax and SCV puncture. Paranchimal lesions were sutured, chest tube was inserted and MVR was postponed. We thought that, previous infection caused pleural adhesion and caused technical difficulties; therefore catheter guide wire punctured the vein wall and caused hemothorax. We emphasize that; ultrasound guidance can be performed during difficult central vein catheterization with high success and few complications.
