Özet

Spinal cerrahide kemik çimentosunun dolaşıma kaçması iyi bilinen bir komplikasyondur. Vena kava filtresi, genellikle alt ekstremitelerden kaynaklanan pulmoner emboliden korunmak için kullanılır. Literatürde kemik çimentosu yakalamış ve çıkartılmamış vena kava filtresi vakası yayınlanmamıştır. Biz bu yayında spinal enfeksiyon sebebiyle korpektomi, debridman ve stabilizasyon prosedürleri uygulanmış olan 84 yaşındaki erkek hastayı sunduk. Hastanın risk faktörleri sebebiyle cerrahi öncesi vena kava filtresi konulmuştu. Cerrahi prosedür sırasında dolaşıma kemik çimentosu kaçağı gerçekleşti ve filtre pulmoner emboliye yol açabilecek çimentoyu pulmoner arterlere giderken yakaladı.Takiplerde çimento parçalı filtre çıkartılmadı. Bu vaka takdimi vena kava filtrelerinin kemik çimentosu kullanılacak olan spinal cerrahi ameliyatlarında çimento embolisinden korunmada önemli rol oynadığını göstermektedir ayrıca kısa dönem takipte filtrenin ne durumda olduğunu ve çıkartılıp çıkartılmaması gerekliliğini sorgulamaktadır.

Abstract

Bone cement leakage to the circulation is a well known complication of spinal surgery. Caval filter usually is used for protecting from pulmonary embolism which is sourced from lower extremities. A case of bone cement caught by caval filter and not removed has not been published. We present 84 year old patient who has a diagnose of spinal infection and underwent corpectomy, debridement and stabilization surgery. Due to risk factors, a caval filter has been placed before surgery. Leakage of bone cement to the circulation, has happened during the surgery and filter caught cement on the way to pulmonary arteries which could be possible cause of pulmonary embolism. Filter with cement has not been removed. Caval filters could prevent pulmonary embolism and cemented filter could be safe if it is not removed. This paper presents the role of caval vein filter on protecting from bone cement embolism during the spinal surgery also the fate of a filter with cement in the short term follow up and to discuss whether it is necessary to remove the filter.
