Düşük akım anestezi eğitiminin kısa dönemde anestezik gaz tüketimi üzerine etkisi

Özet

Düşük taze gaz akımı, geri solumalı bir sistemde CO2 absorbsiyonundan sonra ekshale edilen gazın en az %50'sinin akciğerlere geri gönderilmesi olarak tanımlanabilir. Bu çalışmamızda, düşük taze gaz akımlı anestezi yönetimi eğitiminin anestezik gaz tüketimi üzerine etkisini retrospektif olarak araştırmayı amaçladık. Yöntem: Bu retrospektif çalışma, 05.02.2015-07.03.2015 tarihlerinde genel anestezi altında opere olmuş 86 hasta ile yürütüldü. Çalışma için hastane bünyesinde görev yapan 21 kişilik anestezist kadrosunun yer aldığı bir günlük teorik ve bir günlük uygulamalı eğitim verildi. Uygulamalı eğitimin tamamlandığı günün 15 gün öncesi ve 15 gün sonrasına ait ilgili salondaki tüketim verileri karşılaştırıldı. Çalışmamıza 46'sı erkek, 40'ı kadın olmak üzere 86 hasta dâhil edildi. Bulgular: Eğitim öncesi ve eğitim sonrası gruplarda yaş, boy, kilo, cinsiyet ve ASA skoru açısından anlamlı fark saptanmadı. Bu bilgiler ışığında eğitim öncesi değere göre eğitim sonrasında desfluranın tüketim zamanının şişe (240 ml) başına %47, azot protoksitin tüketim zamanının litre başına %63 uzadığı, oksijen tüketim zamanının ise litre başına %95 uzadığı görülmüştür. Eğitim öncesine göre eğitim sonrasında dakika başına desfluran tüketimi (p=0,001), azot protoksit tüketimi (p<0.01) ve oksijen tüketimi (p<0.01) açısından istatistiksel olarak anlamlı azalma olduğu belirlendi. Sonuç: Günümüzde üzerinde araştırmalar yapılmış ve özellikleri ayrıntılı şekilde ortaya konulmuş konulardan birisi olan düşük taze gaz akımlı anestezi idamesi ülkemiz kliniklerinde pratikte hâlen olması gereken yere varamamıştır. Bu durumun öncelikli nedeninin uygulayıcılardaki eğitim eksikliği olduğuna inanıyoruz. Abstract

Low fresh gas flow may be defined as returning at least 50% of the exhaled gas to the lungs after CO absorption in a rebreathing system. In this study, we aimed to retrospectively investigate the effect of education of the management of low fresh gas flow anesthesia on anesthetic gas consumption.Methods: This retrospective study was conducted on 86 patients who underwent surgery under general anesthesia between 02.05.2015 and 03.07.2015. A total of 21 anesthetists/anesthesiologists working in our hospital received theoretical and practical training each lasting for one day. Anesthetic gas consumptions before and after 15 days from the starting day of education were compared. Forty-six patients (male, n=46, and female, n= 40 were included in our study.Results: Age, height, weight, sex and ASA physical status scores did not differ significantly between groups before and after training . Consumption periods prolonged at indicated percentages for desflurane (47 %, per bottle 240 ml) for nitrous oxide (53% per liter), and oxygen (95 %, per litre). A statistically significant reduction was noted following training in consumption rate per minute for desflurane (p=0.001), nitrous oxide (p<0.01) and oxygen (p<0.01) values when compared with preeducation.
