Özet
Amaç: Rejyonal anestezide sedasyon uygulaması hastanın anksiyetesini azaltır ve konforunu sağlar. Midazolam, hemodinamik stabiliteyi etkilememesi ve solunumu deprese etmemesi özelliği ile anestezide ve yoğun bakımda sedasyon amacı ile tercih edilmektedir. Çalışmamızda rejyonal anestezide sedasyon amaçlı kullanılan midazolamın bolus ve infüzyon uygulamalarını karşılaştırmayı ve sedasyon için en uygun dozu bulmayı amaçladık. Gereç ve Yöntem: Bu çalışmada, aksiller yaklaşım ile brakial pleksus bloğu uygulanacak, elektif el ve ön kol cerrahisi geçirecek, ASA I-II sınıfına giren 40 hasta randomize edilerek 4 eşit gruba ayrıldı: Grup 1’deki (n=10) hastalara midazolam 0.05 mg kg-1 i.v. bolus, Grup2’ deki (n=10) hastalara 0.05 mg kg-1 sa-1, Grup 3’ deki (n=10) olgulara 0.07 mg kg-1 sa-1, Grup 4’ deki (n=10) olgulara 0.1 mg kg-1 sa-1 midazolam infüzyonu uygulandı. Ramsay Sedasyon Skoru (RSS) 4 olması hedeşendi. Cerrahi boyunca istemsiz hareket olup olmadığı, midazolam verildikten gözlerin kapandığı ana kadar geçen süre, tüketilen ilaç ve ek ilaç miktarı, bolus ve infüzyon sonlandırıldıktan sonra RSS 2 olana kadar geçen süre, komplikasyon olup olmadığı kaydedildi. Bulgular: Dört grup arasında demografik veriler, hemodinamik ve solunumsal parametreler açısından fark bulunmadı. Yirminci dakikada 0.05 mg kg-1 infüzyon grubunda 3; 0.07 mg kg-1 infüzyon grubunda 4 ve 0.1 mg kg-1 infüzyon grubunda 7 hastada RSS 4 sağlandı. Göz kapama zamanı en kısa (2.4 ± 0.3 dk) olan grup midazolamı bolus uyguladığımız gruptu. ‹nfüzyon uyguladığımız gruplarda daha fazla midazolam kullanıldığı ve RSS 2 olma zamanının daha kısa olduğu, 0.1 mg kg-1 infüzyon grubunda ise ek ilaç gerekmediği saptandı. Sonuç: Rejyonal anestezi sırasında sedasyon amaçlı kullanılan midazolamın 0.1 mg kg-1 sa-1 infüzyon dozu ile arzu edilen RSS 4 sedasyon düzeyinin elde edilebileceği, her hangi bir ek ilaç gerektirmediği ve kısa derlenme süresi sağladığı; rejyonal anestezi sırasında sedasyon amaçlı kullanılabilecek uygun doz olduğu kanısına varılmıştır.
Abstract
Objective: Sedation during regional anaesthesia reduces anxiety and enhaces the comfort of the patient. Midozalam is prefered drug for sedation by not depressing respiratory functions and not interfering the hemodynamic stability in intensive care units and anaesthesia. In this study, we aim to find appropriate bolus and infusion doses of midozolam for sedation in regional anaesthesia. Methods: In this study, 40 ASA (American Society of Anesthesiologists) I-II patients, who will undergo hand and forearm surgery with brachial plexsus block via axillary approach, were enrolled. Then patients randomized in to 4 equal groups: Group 1 (n=10) recived 0,05 mg kg-1 i.v. midazolam bolus, Group 2 (n =10) recived 0,05 mg kg-1 h-1, Group 3 (n=10) recived 0,07 mg kg-1 h-1, Group 4 (n =10) recived 0.1 mg kg-1 h-1 midazolam infusion. Ramsay sedation score (RSS) 4 is aimed. Involantary movements during surgery, the time for eye closure after drug administration, consumed drug, additive dose requirement, the time from stoping bolus or infusion until RSS reaches 2 and compilications are noted. Results: The demographic characteristics, hemodynamic and respiratory parameters were shown no statistical differences between four groups. At 20th minute, RSS 4 was provided in 3 patients in 0.05mg/kg infusion group, 4 patients in 0.07 mg kg-1 infusion group and 7 patients in 0.1 mg kg-1 infusion group. Eye closure time was shorter (2.4 ± 0.3 dk) in bolus group than the other groups. Midazolam using was higher and the time to reach RSS 2 was shorter in infusion groups Additive drug requirement was not found in 0.1 mg kg-1 infusion group. Conclusion: With midazolam, 0.1 mg kg-1h-1 infusion dose during regional anaesthesia for sedation RSS 4 can be achived, no need for additive drug and provides short recovery time. These findings suggest that this dose is appropriate dose for sedation during regional anaesthesia.
