Özet

Fluoksetin özgül serotonin geri alym engelleyicisi (SSRI)synyfyndan bir antidepresandyr. Rasemik bir kary?ym olup Senantiomeri, R enantiomerine göre farmakolojik olarakdaha aktiftir. Klinik etkisinin serotonin geri emilimininblokajyna ba?ly oldu?u kabul edilmektedir. Güvenlik veuygun yan etki profili kadar, çoklu reseptör afinitesinindü?ük olu?u da fluoksetini ve di?er SSRI'lary trisikliklerdenayyryr. SSGI'lar olasylykla santral sinir sistemindeki 5HT'yiarttyrma yoluyla ajitasyon, anksiyete, uyku düzensizli?i,titreme, seksüel disfonksiyon ve ba? a?rysy ve bulantyyapabilir. Kardiovasküler yan etkiler SSRI'lar arasyndayaygyn de?ildir. Ritim bozuklu?undan ba?ka en fazlabildirilen etki postural hipotansiyonu da içeren hipotansiyondur ve fluoksetin ile görülme orany %29.3'dür.Literatür incelendi?inde, ratlar üzerinde yapylan bir çaly?mada prenatal fluoksetine maruz kalynmasy ile pulmonerhipertansiyon arasynda ili?ki gösterilmesine kar?yn, fluoksetin doz arty?ynyn yol açty?y hipertansiyon olgusuna rastlanmamy?tyr. SSRI'lar içinde sadece essitalopram ilegeli?en bir hipertansiyon olgusu bildirilmi? ancak nedenine yönelik bir açyklama getirilememi?tir. Bu makalededaha önce tany konmu? hipertansiyon ve benzeri kronikbir hastalyk öyküsü ile ek ilaç kullanymy olmayan 48 ya?ynda bir kadyn hasta sunulmu?tur. Hastaya depresyontanysyyla fluoksetin ba?lanmy?, belirtilerinde yeterlidüzelme olmayynca fluoksetin dozu 40 mg'a çykylmy?tyr.Ancak doz arty?ynyn hemen ardyndan hastada tansiyonyüksekli?i meydana gelmi? ve doz tekrar 20 mg'a inilincekan basyncy de?erleri normal seviyelere gerilemi?tir

Abstract

Fluoxetine is an antidepressant of the selective serotoninreuptake inhibitor (SSRI) class. Racemic fluoxetine consists of R- and S-fluoxetine. Its S-enantiomer is pharmacologically more active. Fluoxetine's mechanism of actionis primarily that of an SSRI. Security and favorable sideeffect profile, as well as the lack of affinity for multiplereceptors which are mediated by adverse effects associated with TCA seperate fluoxetine and other SSRIs fromTCA. SSRIs possibly induce agitation, anxiety, sleep disturbance, tremor, sexual dysfunction, headache, andnausea by increasing 5HT in central nervous system(CNS). Cardiovascular side effects are not commonamong SSRIs. Mostly reported side effects, except arrhytmia, are hypotension and its incidence is 29.3%. In theliterature, a study conducted on rats, the relationshipbetween prenatal exposure to fluoxetine and pulmonaryhypertension has been shown, however there is no caseof hypertension following dose increase. In literature,there is no reports abaout relationship between fluoxetine and hypertension. Among SSRIs, there is only onereported hypertension case during essitalopram usageand its causal link was declared to be unclear. Here wereport a 48- years-old woman diagnosed with depression and treated with fluoxetine who has developedhypertension following the dose increase to 40 mg/day.After decreasing the dosage to 20 mg/day the bloodpressure returned to normal range
